Medication Error Reporting Form from QSEN.org

http://qsen.org/tag/patient-centered-care/
The Medication Error Reporting Form can be used to help students think
through the process of medication calculation taught in the lab with potential
patient outcomes as a result of errors directly related to clinical patient safety
in an atmosphere of just culture. Students are given medication calculation
questions and are expected to pass with 100%. Students are also responsible
for passing block meds in a simulation model in the lab appropriate at each
level from freshmen to seniors. When a student miscalculates a medication,
this is treated as a medication error incident. The student must complete a
medication error reporting form. The medication error reporting form
requires the student to calculate the safe and correct dose, investigate what
the medication is commonly given for, what the adverse effects commonly
experienced are, identify safety measures that may help to prevent similar
medication errors from occurring and reflect on how the medication error
reporting form has changed their view of medication calculations and
medication administration to patients. The student must sign the medication
error reporting form taking accountability for the error as a registered
professional nurse would be required to sign a hospital incident (QSEN.org)

Learning Objective(s)

At the completion of this learning activity, the learner will:

The learner will exhibit accountability for medication administration.
(Cognitive, Affective)(Understand, Apply, Analyze, Evaluate)
The learner will evaluate the importance of self-reporting and just
culture for patient safety. (Analyze, Evaluate)
Preparation Required - Medication error form will be a part of the student packet for each semester.
Teacher/Facilitator and Instructions will be provided in both the lab and clinical setting. Forms will

Teacher Reflection

Evaluation of learning can be determined by the documentation and the

students comprehension of the seriousness of the event in addition to any
repeated errors over time. These forms must be completed with a nonpunitive environment to foster just culture.
This activity will help the learner to identify the seriousness of safe
medication administration and the importance of self-reporting and just
culture. This activity represents QSEN for patient safety and collaboration to
remedy any systems issues.
