    A pterygium is an elevated, superficial, external ocular mass that usually forms over the perilimbal conjunctiva and extends onto the corneal surface. It can vary in size from small and atrophic to large and aggressive, distorting the corneal topography and obscuring the optical center in advanced cases. The pathophysiology involves elastotic degeneration of collagen and fibrovascular proliferation covered by epithelium. Internationally, prevalence is higher in lower latitudes with increased ultraviolet light exposure. Pterygia occur more frequently in males and those over age 40, and can cause visual impairment in advanced cases or inflammation.

    Background
    Apterygiumisanelevated,superficial,externalocularmassthatusuallyformsovertheperilimbal
    conjunctivaandextendsontothecornealsurface.Pterygiacanvaryfromsmall,atrophicquiescent
    lesionstolarge,aggressive,rapidlygrowingfibrovascularlesionsthatcandistortthecorneal
    topography,and,inadvancedcases,theycanobscuretheopticalcenterofthecornea.[1,2]

    Pathophysiology
    Thepathophysiologyofpterygiaischaracterizedbyelastoticdegenerationofcollagenand
    fibrovascularproliferation,withanoverlyingcoveringofepithelium.Histopathologyoftheabnormal
    collagenintheareaofelastoticdegenerationshowsbasophiliawithhematoxylinandeosinstain.This
    tissuealsostainswithelastictissuestains,butitisnottrueelastictissue,inthatitisnotdigestedby
    elastase.[1,2]

    UnitedStates
    OccurrencewithintheUnitedStatesvarieswithgeographicallocation.WithinthecontinentalUnited
    States,prevalenceratesvaryfromlessthan2%abovethe40thparallelto515%inlatitudesbetween
    2836.Arelationshipisthoughttoexistbetweenincreasedprevalenceandelevatedlevelsof
    ultravioletlightexposureinthelowerlatitudes.[3,4]
    International
    Internationally,therelationshipbetweendecreasedincidenceintheupperlatitudesandrelatively
    increasedincidenceinlowerlatitudespersists.

    Mortality/Morbidity
    Pterygiacancauseasignificantalterationinvisualfunctioninadvancedcases.Theyalsocanbecome
    inflamed,resultinginrednessandocularirritation.

    Sex

    Pterygiaarereportedtooccurinmalestwiceasfrequentlyasinfemales.

    Age
    Itisuncommonforpatientstopresentwithpterygiapriortoage20years.Patientsolderthan40years
    havethehighestprevalenceofpterygia,whilepatientsaged2040yearsarereportedtohavethe
    highestincidenceofpterygia.
