Background
Apterygiumisanelevated,superficial,externalocularmassthatusuallyformsovertheperilimbal
conjunctivaandextendsontothecornealsurface.Pterygiacanvaryfromsmall,atrophicquiescent
lesionstolarge,aggressive,rapidlygrowingfibrovascularlesionsthatcandistortthecorneal
topography,and,inadvancedcases,theycanobscuretheopticalcenterofthecornea.[1,2]
Pathophysiology
Thepathophysiologyofpterygiaischaracterizedbyelastoticdegenerationofcollagenand
fibrovascularproliferation,withanoverlyingcoveringofepithelium.Histopathologyoftheabnormal
collagenintheareaofelastoticdegenerationshowsbasophiliawithhematoxylinandeosinstain.This
tissuealsostainswithelastictissuestains,butitisnottrueelastictissue,inthatitisnotdigestedby
elastase.[1,2]

OccurrencewithintheUnitedStatesvarieswithgeographicallocation.WithinthecontinentalUnited
States,prevalenceratesvaryfromlessthan2%abovethe40thparallelto515%inlatitudesbetween
2836.Arelationshipisthoughttoexistbetweenincreasedprevalenceandelevatedlevelsof
ultravioletlightexposureinthelowerlatitudes.[3,4]
International
Internationally,therelationshipbetweendecreasedincidenceintheupperlatitudesandrelatively
increasedincidenceinlowerlatitudespersists.
Mortality/Morbidity
Pterygiacancauseasignificantalterationinvisualfunctioninadvancedcases.Theyalsocanbecome
inflamed,resultinginrednessandocularirritation.
Sex
Pterygiaarereportedtooccurinmalestwiceasfrequentlyasinfemales.
Age
Itisuncommonforpatientstopresentwithpterygiapriortoage20years.Patientsolderthan40years
havethehighestprevalenceofpterygia,whilepatientsaged2040yearsarereportedtohavethe
highestincidenceofpterygia.
