Objective Focus (Common Core Standard) - What
standard would match the behavior?
Who and What will occur; How often - What will the
intervention be, with who, and how often?
Indicator of Success (Use SMART goal format) - Write a
goal for the student based on the plan that can be
measureable.
Start Date of Intervention - Date you are starting the plan
Progress Check/Date* - Using the measureable data, report
often teh results of the plan according to the data.
Parent Contact Date - Parents should be contacted any time
a separate plan is put in place for their child. They should
also be contacted if there are changes made.

B. Try intervention.

Record on Form F in Progress Check.

1. Is the plan working?


YES: Continue on STEP 3
Continue plan, , continue data tracking,
and continue recording progress in Form F
in the Progress Check column.

next line."= Go back to STEP 1.
OR
Intervention is working.
"We will continue this intervention."

NO: Go back to STEP 1


Follow the steps again.

interventions, recorded on Form F before
moving to STEP 5.
