3. Nature of Event : Unit withdrawn to attend Economiser tube leakage.

4. Observations :

Suddenly feed flow increased, ID fan loading increased, abnormal sound at (LHS) Economiser
bottom area observed and also water spray observed on LHS of boiler at 34m level manhole,
hence set withdrawn.

5. Analysis

Economiser tube leakage in bottom bank area. Primary failure due to welding joint
failure.

6. Works carried out by maintenance section

Primary failure Economiser coil no.-1, tube no.-6

Secondary failure Economiser coil no.-1, tube no.-7
New distance piece put & welded in tube no. 6&7 hydraulic test taken found ok.

7. Works carried out for the restoration by operation :

Boiler lighted up

8. Recommendations to avoid reoccurrence :

Detail analysis of welding failure will be done. Proper care will be taken in O/H for ensuring
