Mias Profile
Parents/carers expectations

Mias Parents think her moodiness is part of growing up. They are meeting with the School counsellor
to discuss her issues. They will be referred to the community health centre for anonymity as Mia is not
comfortable taking with anyone in school. She will attend the Youth centre resilience outreach program
to build self-esteem with likeminded students.

Health and safety information including a copy of a health plan if applicable

Mia will be required to complete a health care plan with community health (10 free sessions) as a
result of mandatory referral for her self-harm issues. Her issues will be also be discussed with the
wellbeing team and will be nominated by her year advisor to participate in the art therapy program.
She is building a good rapport with the SLSO who is attached to her classes which is a good start for
building mutual trust.
Skills, strengths and interests for learning

Mia has begun to show an interest in Drama. She enjoys drawing and listening to music. Mia was
once involved with the local arts centre but has stopped going this year.
