McBride was assigned as Security Officer in the Special Care Unit. At approximately 10600 hours | looked up at the camera system that was montoring cell Fe-002 McCormackschuler, Jennifer #120745 fand observed Inmate McCormackschuler on the floor. Inmate McCormackschuler is reported to be approximately 4 months pregnant and is on @ Mental Health Observation and Drug Watch. | observed a large puddle of water on the floor and the cell was not sanitary. | immediately went to Fe-002 and asked Inmate McCormackschuler what happened and she stated she had fallen. | moved the bed trat appeared to have her trapped against the wall. | then contacted the Charge Nurse and Zone supervisor to respond to SOU. ‘ [At approximately 0605 hours Sgt. Hinson, Sat. Lemons, Cpl. Vaughn, SRT Washington, Charge Nurse Surratt, and Nurse Dixon arrived on post, Inmate McCormackschiler was assisted to a gray chair and her bed was removed from the cell. Nurse Surratt checked Inmate MeCormackschuler’s vitals and reported them asqaaNBN Nurse Surratt advised that there was no physical sign of injury at ths time, Nurse Surratt asked me if Inmate MeCormackschuler had eaten and | advised her that Inmate McCormacschuler had refused her tray. Nurse Suratt then advised that Inmate McCormackshulers mat would need to be placed on the floor. All responding ares assisted in cleaning up cell and clean linens were issued. Nurse Surratt advised Inmate McCormackschuler to eat her meals and slow down on drinking water. All parties were off post at approximately 0825 hours.
