2 Interpret critical aspects of the persons experience of acute

The acute care setting differs greatly from my previous


clinical experiences in that it is a lot more fast paced and
requires a large amount of charting. A normal shift can
seem chaotic at times, especially around shift change, but a
routine typically exists. First assessment begins at 07000800 where a set of vitals and basic head to toe
assessment occurs; any abnormalities can then be
addressed. I then proceed to get my patients all washed up
with a simple bed bath, which is an opportunity to assess
skin integrity, and have the patient perform oral care.
Breakfast comes at 0800 and first round of meds usually
begins. A second assessment occurs at 1100-1200 paired
with lunch and a second round of meds. Throughout the
day, a variety of experiences can occur. Physio typically
comes to get the patient up and moving which is an
essential element in recovery time. Procedures that can
occur are removal of hemovacs, dressing changes, hanging
saline bags, measuring I/Os etc. In an acute setting,
patients receive very focused care and there is always
nurses working together and lending a helping hand.
On an orthopedic floor, patients may have either had
elected surgery, have a fracture, or ordered by family
doctor. A patient will require surgery is they experience pain
in movement, joint pain, swelling, inflammation, poor
mobility, and dysfunction to the point where it is interfering
with normal activities of daily living. After surgery, patients
typically feel tired, drowsy, and may be nauseous due to
the anesthetic. Pain is different for each individual and will
receive morphine or hydromorphone in the form of a PCA or
receive oral pain medications. If patients overuse their PCA,
they can experience involuntary twitching, drowsiness and
pinpoint pupils. The incision site may be sore and stiff for a
few days post-op, but encouraging walking and exercises
with physiotherapy will allow the patient to get back to
normal and will aid them in getting their strength back.
Being confined to one room for days may be taxing on a
patients emotional health but by simply having a
conversation with them may boost their morale and aid in
the recovery process. Allowing family and friends to visit is
essential because having familiar faces in an unfamiliar
place allows the patient to feel comfortable and less
