WE, PETER A. VELCHES, Filipino, of legal age, single and
resident of Brgy. Tumbod, Municipality of Taytay, Province of Palawan;
and ELA B. SANTOS, Filipino, of legal age, single, and resident of
Brgy. Tumbod, Municipality of Taytay, Province of Palawan, after
having been duly sworn to in accordance with law, hereby depose and
say:

That we personally known NOEL SYCHAN BANIEGA because

we are family friends and we have known hIM for several years;

That we also personally know that NOEL SYCHAN BANIEGA

has been using such name since he was a child;

c.

That we personally know that from available records indicate

that his name is NOEL SYCHAN BANIEGA;

d.

That we attest that the names NOEL SYCHAN BANIEGA, NOEL

SECHIAN BAEGA, and NOEL SYCHAL BANIEGA refer to one
and the same person;

e.

That we are executing this affidavit in order to attest to the truth

of the facts aforementioned, and for any legal purpose this may
serve.
