Prospective students must


submit admissions
applications by the deadline
in order to be scheduled for
an audition.
This scholarship recognizes
volunteer service in the school
and community and does not
have a grade point average
restriction or requirement.
Applicant must be 1) a California
resident, 2) graduating from a
high school in California with a
PTA/PTSA unit in good standing,
and 3) be a member of his/her
high school PTA/PTSA, and able
to submit a copy of your current
PTA/PTSA membership card with
application.
Graduating high school seniors
who are US citizens of
permanent residents. To receive
an award, you must be a full time
student (12+ units) in the 20152016 academic year, in a health
professional program as listed
and be attending an accredited
California college or university.

Graduating high school senior
who is entering freshman year at
an accredited two- or four-year
university, college, or vocational
institute in the fall of 2015 and
who owns/operates an
established business. Many
applicants participate in
organizations such as DECA,
Junior Achievement, Future
Business Leaders of America
(FBLA), etc., but this is not a
requirement. Award recipients
are selected by an independent
scholarship organization.
Scholarships are merit-based,
not based on financial need.
