BLOUNTSTOWN POLICE DEPARTMENT SERVICES EVENT REPORT Information Event Date: Monday, Decomber 21, 2015 Report of Officer John Tadlock (On 12-21-2018 at approx 0446 hrs | was dispatched to Calhoun Liberty Hospital in reference to Barbara Dawson refusing to leave the premises. Upon my arrival contact, was made with hospital staff who stated that Dawson was in room six and was refusing to leave. Staff also stated that Dawson was ok and had been discharged. [At this time | made contact with Dawson and her aunt who was in the room with her. | tried explaining to Dawson that she had to leave the hospital at the requast of hospital staff. As | was trying to explain this to Dawson, she would refuse to ‘comply and also stated that she stil did not fee! well and that she wanted more treatment. told Dawson that she had been discharged and If she wanted more treatment, she would need to go to another facility. Hearing this Dawson continued to refuse to vacate the premises. While refusing, Dawson was acting in a manner which was affecting the peace and quiet of other persons in the hospital {After mutiple verbal attempts to gain compliance from Dawson, she was placed under arrest for Disorderly Conduct and. ‘Trespassing, | attempted to remove the oxygen hose from Dawson but she refused to allow me to take it from her. At this time 2 female hospital staff member told me | could remove it from the port in the wall which I did. At this time | placed handcuffs on Dawsons left hand and attempted to place it behind her back. After a brief struggle and multiple verbal attempts to get Dawson to place her hand behind her back, ! was able to get her left hand behind her back. During this Dawson was Continuing to resist arrestin a non violent but non compliant manner. Dawson also complained of not being able to breathe but she appeared to be breathing fine as she was talking. Medical staff was also present and did not seem to think Dawson was having trouble breathing. After this, | continued to give Dawson verbal commands to place her other hand behind her back which she would not do. After being unable to get Dawsons right hand behind her back, | requested Hospital paramedic Drew Peacock to assist me in handcuffing Dawson. At this time myself and Paramedic Peacock were able to secure Dawsons other hand ‘and complete the handcuffing procedure. (Once Dawson was placed in handcuffs and restrained, she was walked out ofthe hospital and to my patrol car. While walking Dawson to my car, had to push her from behind to get her to go with me. Once at my car, | reached for my car keys. ‘and at this time Dawson began to fall to the ground which was a asphalt surface. | continued to hold Dawson as she fell to the ground approximately 1-2 feet from the back door of my patrol car. Moments later medical staf arived at my patrol car where Dawson was lying. As myself and medical staff rolled Dawson over in an attempt to sit her upright, she sustained cuts. (on her feet and knees as she came to a resting position on her knees. It was believed by myself that Dawson was just being non compliant and making herself dead weight in an effort to avoid going to jal. Myself and medical staff made several ‘attempts to place Dawson in my patrol car to no avail | continued to ask Dawson to comply with my request and get in the car but Dawson continued to be un responsive to any commands or request. | then asked medical Staff (Jennifer) who is a nurse to ‘check Dawsons vitals which she did. Jennifer advised that Dawsons vitals were normal with a pulse of 70 and a pulse ox of 97% Medical staff also asked that the handcuffs be removed from Dawson so a more accurate vial reading could be taken. At this request | removed one side of the handcuffs freeing and allowing Dawsons hands to come infront of her. More attempts were ‘made to get Dawson in the back of my car to no avail. | then contacted dispatch to see if the jll had a van or if EMS could transport Dawson to the county Jal. Also while waiting for an answer, | continued to have medical staff check Dawsons vitals Which they did and assured me that she was within normal range. Moments later @ Doctor walked outside and checked Dawsons vitals. After doing so, he stated that Dawson needed to be taken back inside the hospital, | asked the doctor if Dawson was being readmitted and he said yes. He stated that she was, being readmitted for symptoms totally different than what she had been released from earlier. At this ime a stretcher was brought outside and myself and medical staf fted Dawson onto the stretcher. Dawson was then taken inside and medical staf began treating Dawson. At this time, | was stil in the room with medical staff and Dawson and it appeared to me by the ‘treatment she was receiving that Dawson was in declining health. | asked a staff member if Dawson was critical and she asked the Doctor who replied yes. Moments later Captain Terry arrived and asked ‘me what had taken place. After answering Captain Terry, he ordered me to retumn to the station and complete an incident report of the situation which | did. These incidents did occur in Calhoun County FI, City of Blountstown and are in violation of F.S.S. 810.08 Trespassing and F.S.S. 877.03 Disorderly Conduct. Al incidents were recorded via audio from my in-car camera mic. Nothing Follows at the current time, Erne! Tery) a received a call from Officer John Tadlock at approximately 6:30 a.m. He advised that he was ‘at the hospital and had arrested Barbara Dawson for Trespassing and disorderly conduct. He advised he had been walking her to the car and that when he reached for his keys she collapsed in the parking lot. He advised that medical staff had gotten her back in the hospital and they were currently doing CPR. | arrived at the hospital at approximately 5:46 a.m. and met with Officer Tadlock who advised that Barbara Dawson was transported via ambulance complaining of abdomen pain . He advised that she had been treated and cischarged from the hospital but she refused to leave. He advised medical staff had called for her to be removed by the Police Department . He said when he arrived he tried to talk Ms. Dawson into leaving but she refused to go and became disorderly so he arrested her for Disorderly conduct and trespassing. He advised he tried to cuff her but she resisted without violence and he asked Paramedic Drew Peacock to help him cuff her. After she was cuffed he was walking her to his car and she would stop and was arguing . He advised he had to keep pushing her forward to keep her going and she was highly agitated . Once outside near his patrol car Ms. Dawson collapsed and went to the ground . Ofc Tadlock advised that mecical staff was checking her and sid she was fine and then a short time later Dr. Stewart Warren came out and checked her and had them bring her back in ‘the hospital. | could see that they had Barbara Dawson in a trauma room and were doing CPR. Officer Tadlock confirmed that his in car camera was recording and he had hiis body microphone activated. | had him go to the office and start a report. ‘AL5:58 a.m. Investigator Partridge arrived on scene to assist. | asked him to handle requesting video from the hospital ‘and get a copy of the in car video for me ‘At6;24 a.m. Doctor Stewart Warren pronounced Barbara Dawson deceased. ‘At6:54 a.m. | made contact with Investigator Whit Majors of the medical examiners office and he took jurisdiction of the body. Peavy funeral home was the rotation funeral home and Jerry Kollark transported the body to the medical examiners office. ‘The following isthe list of persons that were present atthe hospital and | took sworn recorded statements from all of them. Haylee Harris Revell - admissions clerk ‘Stewart Warren - Doctor Jessica Larkins Brady - xray tech Rachel Crawford -CNA Jennifer Waldorf E.R. R.N, Karen Taylor - Floor R.N. Drew Peacock- Paramedic Nathaniel Batto- was in lobby ‘Angela Donor- aunt of Barbara Dawson Allof the interviews were consistent that Barbara Dawson was brought in via ambulance with a complaint of abdominal pain ‘and was treated and discharged . All say that Ms. Dawson was not happy when she was discharged and wanted to stay at the hospital and became unruly. Officer Tadlock arrested her and walked her out the door and she collapsed near the car Haylee Revell was outside when Ms, Dawson collapsed and she went and got R.N. Jennifer Waldorf to come check her vitals, Jennifer Waldorf said she went outside and checked her and found that she was breathing and her oxygen saturation was 98 percent. She checked her a few minutes later and she was stable and breathing. Dr. Warren came out and checked Ms. Dawson ‘and had her moved back inside where he found she was not breathing and started CPR. The doctor pronounced her deceased at 6:24am. Investigation is stil open pending results of Autopsy. Page 6 BLOUNTSTOWN POLICE DEPARTMENT SERVICES EVENT REPORT ATTACHMENT Information Event Date: Monday, December 21, 2015 SE ‘Acopy of Ms. Dawson's medical chart from this visits included in this fle @ long with disc's with statements and dash camera. ‘Afier the family was allowed to see the body the alleagtions were made by the unknown femy member that Ms. Dawson had been dragged tothe patrol car. Ms. Dawson had an abrasion on her toe and on her knee. Nobody that was interviewed said anything about her being dragged to the car. I is possible that her knee and toe got abrasions when she collapsed on the asphalt. Investigator Partridge met with Steve Cherry of Calhour-Liberty hospital and he advised there was no video because there server had been down for 2 weeks. Investigator Partridge spoke with Don at Kingdom Hall church and he advised they do have video but it does not flm any of the hospital property | (Capt Terry) spoke with Kelly Allen who is the manager of Chipota manor apartments and they have no video on their property.
