AFFIDAVIT

I, SADAMU WATANABE, of legal age, married/single, presently

residing at ___________________________________, after having
been duly sworn to in accordance with law do hereby depose and
say that:
I am the biological father of Yugi Watanabe, born on May 13,
1988 born at Naguimba, Banayoyo, Ilocos Sur, Philippines;
I and his mother namely, Maribel Basilio did not contract into
marriage here and in abroad prior to or even after the birth of Yugi
Watanabe;
I am a Japanese citizen/national since birth and I never
renounced neither do I applied for naturalization of foreign
citizenship to relinquish my Japanese Citizenship/Nationality; and
I am a Japanese citizen/national when Yugi Watanabe was born
up to present.
I am executing this affidavit to attest to the truth of the
foregoing statements and for all legal intents it may deemed
served.
