As previously noted, masonry walls should be restrained as shown on the construction drawings. 3. _Itwas previously documented that Amec Foster Wheeler has become aware of the contractor "remediating" the shear wall reinforcing in ways which do not match the construction drawings. Documentation of contractor's remediations has not been received for review. See Photo's 2 to 6. 4. Some areas still require honeycombing remediation. See Photo 7. 5. Openings were cut into one of the masonry shear wall’s in the Penthouse. Ver Reinforcing was cut during this process. See Photo's 8 to 12. 6. Opening cut into shear wall allowed opportunity to view lack of vertical reinforcing within the shear wall. This wall is along gridline G between gridlines 7 and 11 on the ground floor. The following items were noted during the site tour on Tuesday May 19”, 2015: Structural 1. Bond beam and reinforcing around openings apparently not installed in elevator masonry walls. See Photo 1 2. Aconcem has previously been raised that bond beams have not been installed in some locations during the installation of masonry walls. it is not possible for Amec Foster Wheeler to visually verify this during the site visit since only the face of the masonry block is visible. 3. _Inaddition to ensuring the bond beams are installed as per the construction drawings, the top courses of masonry must be grouted where specified on the construction drawings. It is not possible for Amec Foster Wheeler to visually verify this during the site \isit since only the face of the masonry block is visible. 4. As previously noted, masonry walls should be restrained as show on the construction drawings. Contractor should submit stamped sketch showing existing detail works or a ‘stamped sketch showing proposed remediation. This is a major concer as finishes are being installed and this will be difficult and potentially costly to remediate afterwards. See Photo's 2 and 3. 5. Column to shear walll interface appears to have insulation in it. See Photo 4. PMF.06 Rev. 1Project Site Report Report Date: June 12", 2015 Page 2 6. 10. "1. 12, 13, 14, 15. In Photo 5 it appears there are multiple broken (and at least one intact) masonry strap anchors which are visible. Contractor to state if masonry strap anchors which were broken were replaced with additional masonry strap anchors. Comments or remediation detail has not been received from contractor for previously noted deficiency where contractor appears to have failed to install masonry strap anchors in shear wall at column F7. Could not access music room D-104. See Photo 6, Itwas previously documented that Amec Foster Wheeler has become aware of the contractor "remediating" the shear wall reinforcing in ways which do not match the construction drawings and have not been submitted for approval. See Photo 7. As previously noted, installation of additional reinforcement at openings in concrete block walls does not match construction drawings. Contractor has begun “remediating” this issue without providing stamped drawings for review. See Photo 8. Contractor must. provide stamped remediation for review before proceeding to implement in other areas. See Photo 9. ‘Some areas still require honeycombing remediation. See Photo 10. ‘As previously noted, it has come to the attention of Amec Foster Wheeler that the library curtain wall which was modified as part of CCN No. 98 is being installed but does not match the issued drawings. Contractor still has not submitted stamped procedure/drawings for this remediation. As previously notes, shear wall along gridline A being covered with finishes despite incomplete Shear Wall construction and incomplete required remediations. For example, the column to shear wall connection. See Photo 11 As previously noted, bond beam installation along gridline G between gridlines 7 and 11 does not match construction drawings. Contractor has notified Amec Foster Wheeler that remediation of the bond beams along gridiine G has been completed and the wall has been constructed as per the drawings. Contractor did not submit a stamped procedure or drawing(s) for review prior to commencing work. It appears that the contractor has removed between one to three courses of masonry in order to install the bond beam. The contractor may have had to cut the vertical reinforcing in order to complete this. This would not match the contract drawings and requires remediation. As previously noted, it appears that insufficient clip angle contact is visible in various areas throughout the building. The procedure described involves dropping reinforcement from the top of the wall into the appropriate wythe without providing connection to the slab or the required overlap. The Sopraseal membrane must have rigid backing material to which it can be adhered. This item was noted in previous reports. There is insufficient rigid insulation at the base course of concrete block on the exterior wall along grid line 1 between grid lines A and H (see photos 186 and 190). Contractor should ensure that there are no areas where insulation is missing. ‘There are several locations where the mortar joints have not been struck flush (see photos 187 and 188 showing joints on the West side of area D along grid line H between grid lines 1 and 2). This appears to have prevented the rigid insulation from being installed tight to the concrete block and membrane reduced the depth of the air cavity between the masonry veneer and the rigid insulation and caused the insulation to be damaged when the retaining wedges were installed (see photos 184, 185, 188 and 190). These conditions are not acceptable. This item has also been noted in previous reports. There are several locations where the Sopraseal membrane is wrapped around steel! columns on the exterior wall and spanning the web from flange to flange with no backing material (see photo 209). This is unacceptable and the membrane must hate rigid backing material. This item was also noted in previous reports. PMF.06 Rev. 4Project Site Report Report Date: November 21#, 2014 Page 2 5, There is reinforcing steel projecting up from the concrete slab in the shear wall opening at the entrance to A143 Home Economics, on grid line Q between grid lines 27 and 28 (see photo 037). There should not be any steel reinforcing projecting from the slab in this location. Contractor should verify that the opening in the shear wall is the correct size and is correctly located. There are missing masonry ties and insulation retaining wedges along grid line H between grid lines 1 and 2 (see photos 191, 192 and 198). This item has also been noted in previous reports. There are alternating bullnose and square edge concrete blocks around the jambs of windows WO3 and WS in rooms C101 and C109 (see photos 102 and 103). The vertical face of the brick veneer at the roof line below the top of the parapet is not parallel with the vertical face of the parapet (see photos 1443 and 1493-courtesy Leon Murphy, Transportation and Works). In some locations the parapet overhangs the brick veneer by approximately 50mm and in other locations, in some cases along the same wall, the vertical face of the brick veneer is outside of the vertical face of the parapet. This is unacceptable. Report The following are my concerns and observations noted during the review: 1, Itappears that there is no rigid backing material behind the Sopraseal membrane covering the columns not requiring a fire rating (see photo 046). The Sopraseal membrane must have rigid backing material to which it can be adhered. 2. The inner layer of insulation is missing between windows at intersection of grid lines T and 19 (see photo 039). Contractor should ensure that there are no areas where insulation is missing. 3. There are several locations, particularly on the South side of the building, where the mortar joints have not been struck flush (see photos 045 and 048). On the exterior wall of stairway S-4, adjacent to the library curtain wall, the Sopraseal membrane has been installed over mortar joints that have not been struck flush (see photo 054). These conditions are not acceptable. 4. There are several locations where the Sopraseal membrane is wrapped around steel columns on the exterior wall and spanning the web from flange to flange with no backing material (see photos 032, 058, 061, 064, 066, 067, 082and 313). This is unacceptable and the membrane must have rigid backing material. PMF.06 Rev. 1Project Site Report October 20", 2014 Page 2 5. ‘The Sopraseal membrane and metal flashing installation on the comer at the intersection of grid lines G and 14 is unacceptable (see photos 57, 58 and 59). 6. The damaged flashing adjacent to door X17 in room D101 is unacceptable and must be replaced (see photo 4874), 7. There are several locations on the exterior wall where it appears that the rigid insulation, retaining wedges have not been installed (see photos 4878 and 4884). 8 The rigid insulation on the exterior wall does not extend all the way to the masonry check on the North side of the building on either side of door X15 (see photos 084 and 085). This condition is unacceptable. 9, There is 3” of rigid insulation on the exterior walls on grid line 9 and grid line H (see photos 095 and 096). The contract documents indicate 4” ofrigid insulation. 10. The concrete sill return on doors X19 and X20 jambs are filled with brick remnants and rigid insulation (see photos 110, 115 and 116). This gap should be filed with concrete sill material matching the concrete sil. 11. The infil of the column base plate check in the foundation with various masonry materials of different colours and shapes is unacceptable (see photos 113, 114, 119, 120 and 122). 12. There are gaps in the sealant on the gypsum board covering the steel beam supporting the roof along grid line H (see photos 198 and 199) 13, There are two rooms (A224 and A226) on level 2 in which the spray applied fire stopping material on the steel beams supporting the foor of level 3 appears to be wet (see photos 259 and 261). 14, The Sopraseal membrane on the exterior wall adjacent to and around the windows in rooms A301 and A303 is melted, and in at least one location, burned through (see photos 313, 317, 318, 319 and 321). 15, There was no barrier restricting access to the roofof level 3 and no indicator or barrier restricting access to the area within 2 metres of the edge of the roof (see photos 350 and 351). As per my conversations with Robert Rideout, the washroom trailer has to be moved to install the Concrete Sitting Walls on the SW corner of the school. Observations Noted During the Site Visit ‘+ Weirs are installing the forms for the sidewalks on the North side of the Gym and Cafeteria. Manhole 3703R is installed +/- 1.5 meters to far to the North therefore the Manhole is partially in the asphalt (see pic 4855 attached). | contacted Weirs on same and the Manhole will be adjusted as required and the asphalt extended to surround the cover to eliminate erosion. © As per Section 01 52 00 Item 1.13.1 of the Specifications, construction debris, waste materials and packaging materials are to be removed from the work site on a daily basis. There is a lot of construction related debris scattered over the site especially on the East Elevation (see pics 4870, 4871 & 4873 attached). «The entrance to the site is basically completed except for the new traffic lights and some landscaping ete .(see pic 4861 attached). However, there was no evidence of this being done at time of site visit. ‘* Contractor was instructed to install supports on vertical piping. However, we received correspondence that this was not required. We therefore request that the contractor engage the manufacturer's representative to visit the site and provide a written report on the installation of drainage, venting and rain water piping. Also, include report on AquaRise CPVC piping for domestic water piping. ‘+ Domestic hot water tanks have arrived on site, 2. Ventilation ‘¢ Itwas note that round spiral ductwork sitting on the floor on the third level contained excessive amount of construction dust. Ductwork should be covered at alltimes. ‘Ductwork for Areas C and D has arrived on site and contractor was in the process of sorting and fabricating ductwork on site. * Contractor has not provided documentation on the cutting of flanges for the roof curbs to accommodate the 38 x 93 mm wooden pressure treated roof curbs as previously requested. It was also, noted that there is no plastic or wire mesh protecting the fibreglass insulation. This as well requires clarification from the roof curb manufacturer. On-going no report. + The installation of the of ductwork is ongoing on the 2™ Level corridor ceiling and the connecting onto the VAV Boxes and Crosstalk silencers is ongoing Area A and B. ‘+ Itwas noted that Insulator was not installing welded pins on the ductwork as per Specifications Section 23 07 13. Previous duct insulation installed is to be removed and pined and reinstated. + AMEC advised that Vanne Technical Services Inc, had performed the Duct Leakage Testing. AMEC advises that the Balancing Company does not meet Specifications Section 23 05 93 and the Duct Leakage test will have to be reapplied meeting the requirements of Specifications. On-going no reply to date. 3. Fire Protection ‘* _Itwas noted that the sprinkler piping for the gymnasium rises up and goes down again where it passes through the stage area. Contractor to advise on how future modifications or repairs are going to know that when removing a sprinkler there be more than 5 gallons of water trapped when removing or repairing a sprinkler head. * Contractor has installed sprinkler piping in the Gymnasium at time of site visit. + Water entries for the domestic water and fire protection have been installed outside the building and contractor is in the process of installing pipe support rods on the inside of the building in Mechanical Room D116. However, this piping is incomplete and additional information is to be provided. Still outstanding. This is a structural concem and the remediation should be reviewed and approved by the Structural Engineer(s) prior to installation. See Photo's 5 - 8 showing an example which was noted in previous reports and has not been remediated but has been partially covered up with masonry. Performed one spot check to establish if previously noted missing and damaged bridging has been fixed. It was noted that in this area the bridging has not yet been completed. See Photo 9. . At the time of this site visit some portions of lower masonry remediation were visible. See Photo 10. These portions that were visible appear to be acceptable according to the remediation detail. As discussed inspection of masonry remediation is required. (See Item 6) ‘As mentioned previously, “It was previously noted that the lower masonry remediation detail had not been installed properly. Contractor has begun installing exterior block (between gridlines 28 and 23 along grid line P) without contacting AMEC for an inspection of masonry remediation prior to installation of the exterior block. As mentioned previously, “Large areas of ponding observed on concrete floor in Area C. See Photo 11”. MARCO responded with, “Slab within tolerances”. MARCO to provide inspection documentation/results for review. 8. As mentioned previously, “Masonry remediation detail has not yet been installed along gridline T between gridlines 18 and 22. Please see photo 10 from May 9", 2014 showing that at this time the masonry had been installed along this portion of the building. Considering the masonry remediation detail was issued May 20”, 2014 it is assumed that this portion of the wall requires remediation. Please provide documentation if for some reason this specific area does not require remediation. * MARCO responded with, “These walls were constructed as per drawing after issue was discovered”. MARCO to provide documentation (i.e. photos taken during construction of the wall) for review. 9. As previously mentioned one coat of primer is to be applied to all steel surfaces except as noted for specific situations. As shown in photo 11 and 12 the HSS Brick support structure requires painting and the exposed steel columns in Area B require repainting. 10.
