have been employed at MQM since 1996, I am the leed Structural engineer and Project Manager
for MQM Quality Manufacturing Ltd. Since May 28", 2009, I have been the Welding engine
and Quality Assurance Manager for MQM Quality Manufacturing Ltd.
One of my responsibilities at MQM, is to develop, prepare and implement new and existing
welding procedures to our shops and field. These MQM’s welding procedures and other
prequalified procedures have been used on the West End High School project.

Observation:
The following site review observations were recorded and discussed with David McCall and the
contractor:
1. Safety Issues: NA
2. Lateral supports were not installed as per typical steel details on drawing S1 (attached)
at many concrete block walls. Sample photos and descriptions as follows:
a. Photos #1-3 Block walls separating classrooms (Approximately 54 walls & 324
supports).
b. Photo # 4: Block walls at stairwells (typical at each stairwell).
. Photos #5-6: Holes cut in block and vertical rebar was welded to the underside of
steel on gridline 7 to support the top of the concrete block walls (David McCall
said that this method was used at various locations to remediate the missed
installations where there are concrete block shear walls). AMEC Foster Wheeler
(AFW) has concerns about the integrity of this work because:
