Dear Senator Sanders:
This letter summarizes your general health history and current medical evaluation
prepared at your request. My office has served as your physician for the past 26 years. I am
certified by the American Board of Internal Medicine in Internal Medicine, Hematology, and
Medical Oncology.
Your most recent physical examination was on November 18, 2015 when you were 74
years old, 72 inches tall and weighed 179 pounds. Your general physical examination was
normal including a blood pressure of 136/81 mmHg, and a pulse of 72, Comprehensive
laboratory test results indicated total cholesterol of 196 mg/dL, with an LDL cholesterol of 125
mg/dL and an HDL cholesterol of 46 mg/dL. Your thyroid metabolic profile and complete blood
count was normal. Your fasting blood glucose was 94 mg/dL. Your electrocardiogram was
normal,
Over the years, you have been treated for medical conditions including gout, mild
hypercholesterolemia, diverticulitis, hypothyroidism, laryngitis secondary to esophageal reflux,
lumbar strain, and complete removal of superficial skin tumors. Your past surgical history is
consists of repair of your left and right side inguinal hernias by laparoscopic technique, and a
right true vocal cord cyst excision. You have no history of cardiovascular disease.
‘Your current medications include daily levothyroxine, and intermittent use of
indomethacin, You do not use tobacco and use alcohol infrequently. Your most recent
colonoscopy was normal. You are up-to-date on recommended vaccinations.
You are in overall very good health and active in your professional work, and
recreational lifestyle without limitation.
