At the conclusion of Sydney's first 6 weeks


she has impressed myself as well as my
fellow inter-disciplinary team in regards to
her effective, professional communication
and ease with which she has integrated
herself into the team of health care
providers. Sydney thus far has
demonstrated promptness both in regards
to arriving on time for work and addressing
veteran consults and needs. Sydney has

displayed a level of initiative and willingness


to learn that I would rate as typically not
seen from a level II FW student completing
their first rotation in the field. She definitely
has a passion to serve veterans and this is
transparent while observing her interactions
with the veterans she serves. Sydney is not
afraid to ask questions and also has not
shied away from expressing areas of
practice that she has not had much
education or experience with, which I find
commendable. In reviewing her current
performance, I anticipate that Sydney will
be able to handle the full duties of my
position during her last 6 weeks here under
the supervision and guidance of myself and
our staff.

Sydney demonstrated inpatient skills that


are at a level of an entry-level practitioner.
She possesses above average
communication skills and above average
knowledge of the anatomy and disease
processes she has come across during her
time here at the VA. It was a pleasure
having a student with such an exceptional
work ethic, level of professionalism, and
initiative. She demonstrated promising
growth in regards to her knowledge base
throughout this rotation and always asked
insightful questions regarding any difficult
situations. She showed a willingness to
learn and really interacted well with the
veterans she treated. Again, her social and
communication skills were excellent in this
regard. Sydney was able to manage a full
case load under my supervision, was quite
independent with developing and
implementing treatment plans, and
collaborating with the entire interdisciplinary
team. In my discussions with nursing, case
management, PT, and physicians, all were
impressed and saw her more as a team
member rather than a student. Sydney
would be a welcome addition to any VA
PM&R staff in the future.

Sydney has met these standards through


midterm. In contrast to the private sector,
therapists in the VA are not held to strict,
often times unattainable productivity
standards. This has allowed myself to teach
and mentor Sydney on a continuous basis
as I am not having to see patient after
patient after patient. Here, Sydney has the
ability to actually reflect upon an encounter,
discuss it with either myself, another OT, or
PT, and document without having to worry
about productivity or having to rush to
another veteran. This has allowed her to
truly meet the above standards through
week 6.

From an inpatient, acute care standpoint,


Sydney is meeting these standards thus far.
Her documentation is concise, clearly

articulating the therapeutic interventions


and responses to treatment. Again, having
had the time to sit down and truly educate
Sydney in regards to VA documentation
and what is expected with each type of
note, she has quickly attained a good
understanding of the depth and breadth of
documentation. Over the last 6 weeks, I will
look to potentially have Sydney utilize more
assessments/screenings in the acute
setting.

This aspect developed into a strength by


the conclusion of this rotation. She really
did impress me in regards to her
documentation, thoroughness of
evaluations, and dedication to the vets she
served. Our chief of physiatry was
impressed with how well Sydney was able
to evaluate and develop appropriate
treatment plans in a concise manner as well
as offer guidance to the attending
physicians and physiatrists in regards to d/c
recommendations.

Sydney is very personable, easily


approachable, and a compassionate
individual. These traits are vital in
developing rapport with clients, especially
veterans. She has displayed advanced skill
in regards to documentation not typically
seen for a student completing their first
level II FW. Very good knowledge of FIMS.

Exceptional. Many students struggle with


this aspect. However, from early on Sydney
displayed a willingness, and confidence in
interacting with the interdisciplinary team as
well as the vets. Both written and verbal
communication were exceptional and will
be a strong asset for Sydney as she
continues forward. She really took on the
role of a staff therapist during the second

half of the rotation, which was something I


was looking for her to do.
