ISHIKAWA CAUSE & EFFECT

ANALYSIS
(FISH BONE DIAGRAM)

INTRODUCTION
Ishikawa diagrams (also called fishbone diagrams, herringbone diagrams, cause-and-effect diagrams, or
ishikawa) are causal diagrams created by kaoru ishikawa (1968) that show the causes of a specific event.
Common uses of the ishikawa diagram are product design and quality defect prevention to identify potential
factors causing an overall effect.
Each cause or reason for imperfection is a source of variation.
Causes are usually grouped into major categories to identify these sources of variation.

When to Use a Fishbone Diagram?


When identifying possible causes for a problem.
Especially when a teams thinking tends to fall into ruts.

Fishbone Diagram Procedure


1. Agree on a problem statement (effect). Write it at the center right of the flipchart or whiteboard. Draw a box
around it and draw a horizontal arrow running to it.

2. Brainstorm the major categories of causes of the problem. If this is difficult use generic headings:
Methods
Machines (equipment)
People (manpower)
Materials
Measurement
Environment
3. Write the categories of causes as branches from the main arrow.

The hospital is facing immense problem to ensure sound supply chain management.
There is no coordination among every action and everywhere there is haphazardness.
There are other problem areas as well like staffs are not linked in the system, peoples are
unwilling to share information and people are direction less
As a result the hospital is increasingly failing to maintain its well established and long lasted
reputation

The hospital also can think about hiring IT and technical experts so that those specialists can
guide them about purchasing new equipments and also how to use those up to the highest extent
The hospital need to adopt an Electronic ordering system.
For resolving employee issues they can undertake thorough training campaign to motivate them
and train them in proper way. Coordination among all the employeesalso needed to be done and
it is also require to specify about their individual responsibilities.
The hospital can adopt centralize ordering system to make ensure availability and eliminating
duplicity of order.
In order to solve the environment problem there is no better way other that involving employees
in creating and maintaining work schedule and working standards.
