AFFIDAVIT
I, LEVY LAMPARERO GALILEA, Filipino citizen, of legal age, with the
address at 7 Austin Street, Greenview Village, Las Pinas City, after having been sworn in
accordance with law, hereby depose and say:
1. That my name is LEVY LAMPARERO GALILIA as stated and registered in
my birth certificate issued by office of the Local Civil Registrar of Cabanatuan,
a copy of my birth certificate is hereto attached and marked as (Annex A).
2. That my name LEVY LAMPARERO GALILEA as indicated in my drivers
license, a copy of the photocopy of my drivers license where my name appears
is hereto attached and marked as (Annex B).
3. That LEVY LAMPARERO GALILEA was named and baptized on February
21, 1970 at Brgy. Tiring, Cabanatuan, Iloilo, as evidenced by the certificate of
baptism hereto attached as (Annex C).
4. That the names LEVY LAMPARERO GALILIA as appearing in the
certification (Annex A) and LEVY LAMPARERO GALILEA as appearing
in the drivers license as (Annex B) refer to one and the same person, the
herein Affiant.
5. That the errors were probably committed through inadvertence, honest
mistake or excusable negligence.
6. That on June 3, 2000, I received a certification granting my request for
correction of name, date of birth and Civil Status by Leticia G. Barbers branch
head of Social Security System (SSS), a copy of the certification hereto
attached as (Annex D).
7. That I am executing this affidavit to attest to the truth of the foregoing and to
support my application for loan with the Social Security System (SSS).
