    RELEASE,WAIVE,DISCHARGE,andCOVENANTNOTTOSUEWanderingProductions,
    CottonwoodHighSchool,AcademyforMathEngineeringandScience,&theGraniteSchoolDistrict,
    anyoftheirofficers,employees,agents,volunteers,oremployees(hereinafterreferredtoasthe
    RELEASEES)fromanyliability,claims,demands,actions,andcausesofactionrisingoutoforrelatedto
    anyloss,propertydamage,damage,orinjury,includingdeath,thatmaybesustainedbyme,ortoany
    propertybelongingtome,whileonthegroundswheretheactivityisbeingconducted,andwhile
    participatingintheactivity,REGARDLESSOFWHETHERSUCHLOSSISCAUSEDBYTHE
    NEGLIGENCEOFTHERELEASEES,orotherwise
    1. Agreementtofollowdirections.
    Iagreetoobserveandobeyallpostedrulesandwarnings,and
    furtheragreetofollowverbalinstructionsgivenbyCottonwood/AMESorWandering
    Productionsstaffandadministration.
    2. AssumptionoftheRisksandRelease.
    Irecognisethereareminimalrisksassociatedwithmy
    participation,butintheeventofanyinjury,loss,ordamagearisingfromparticipation,Iassume
    fullresponsibilityformyself,andfurtherreleaseanddischargeCottonwood/AMESand
    WanderingProductionsfromallresponsibility.
    3. Indemnification.
    IagreetoindemnifyanddefendCottonwood/AMESandWandering
    Productionsagainstallclaims,causesofaction,damages,costsorexpenses,includingattorney
    feesandotherlitigationcosts,whichmayinanywayarisefrommyuseoforpresenceuponthe
    facilitiesofCottonwood/AMESandWanderingProductions.
    4. Fees.
    IagreetopayforalldamagestothefacilitiesofCottonwood/AMESandWandering
    Productionscausedbyanynegligent,recklessorwillfulactionsbyme.
    5. ApplicableLaw.
    Anylegalorequitableclaimthatmayarisefromparticipationshallberesolved
    underUtahLaw.
    6. ProvisionfortheMinor.
    IfIamunder18yearsofage,itisgrantedandunderstoodthatmylegal
    guardianisadditionallyresponsibleforalltermsandconditionslistedabove,andwillbeheld
    responsibleforanywillfulactiononmypartandanyfeesincurredunderprovisionoftheabove
    terms.Thiswillbeacknowledgedandagreedtobythesignatureofmylegalguardianinaddition
    tomyownundertheConsentformbelow.
    7. Consent.
    Iconsenttoparticipatinginfilmingof
    St.Charlie
    andagreetothetermsandconditions
    ofthisRelease.Ifundertheageof18,mylegalguardianrepresentsbysigningbelow,thats/he
    haslegalauthorityandcustodyoverme.
