1.TheapplicantshouldbethepermanentresidentofthestateandshouldbethecitizensofIndia.
2.Theapplicantfamilyshouldbelivingandcookingseparately.
3.Theapplicantfamilyshouldnotpossesthefamilycardfromtheotherstate.

Howtosubmittheapplication:
1.IfyouarearesidentofChennaicityandinandaroundareas,gototheAssistantCommissionerofCivilSupplies
oftheparticularzoneandaskfortheNewrationcardapplicationform.
2.IfyouarearesidentofadistrictsotherthanChennai,TalukSupplyOfficersoftheCivilSuppliesandConsumer
ProtectionDepartmentorAssistantRationingOfficers(inCoimbatorecity)concernedoftheareainwhichthe
applicantlivesiscompetenttoreceiveapplicationsforfamilycards.
3.Afterfillingtheapplication,followingdocumentstobesubmitted:
>Proofofapplicant'scurrentresidenceinTamilNadu(copyoforiginaldocument)
AnyoneoftheBelowProofs:
>ElectionPhotoIdentityCard(EPIC),
>Propertytaxpaymentreceiptincaseofownhouse(currentyear),
>ElectricityBill(lastmonth),
>TelephoneBill(lastmonth),
>FrontPageofBankPassBook,
>Passport
>CurrentlyvalidLeaseagreement(incaseofrentedhouse)
>FamilyPhotograph(4copies)
4.Submitthedocumentswiththeaboveproofandtaketheacknowledgereceipt.
5.Thefieldinspectorswillcomefortheverificationtoyourresidencetocheckallthegivendetailsarecorrect.
6.Youcanaskfortheidentityproof,beforeverifyingthedetails.Andthereisnobribetobepaidtothe
Inspectors.
7.Ifalltheabovedetailsarecorrect,theapplicationisapprovedandsendforprinting.
8.Within60daysyourwillreceiveyourcard.

FeesForRationCard:
1.AnamountofRs.5/asthefeeforanewrationcard.ThisamounthastobepaidtotheACorTSOoffice.
2.ApplicantsarerequestedNOTtopayanybribestoanystafforbrokers.

ProcedureofInclusion/DeletionofFamilymembersinRationCard:
ToAddorDeleteanyoftheFamilymember'snameinRationCard,applicanthastofollowthefollowingbelow
process:
1.FilltheformgivenbytheofficeofDistrictFoodandSuppliesController/AssistantFoodandSuppliesOfficer/
InspectorFoodandSuppliesORIntheconcernedTehsil/DistrictLevelinanyoftheworkingdays.
2.SubmittwogroupphotosoffamilyalongwiththeApplicationform.
3.SubmittheoriginalRationCardalongwithanyoneofthebelowdocuments:
a)SurrenderCertificate(Incaseofmarriageofwife/daughterinlaw)
b)BirthCertificate(Incaseofnewlybornchild)
c)DeathCertificate(Incaseofdeathofthememberofthefamily)
4.Aftersubmittingtheapplicationwiththeabovementioneddocuments,theapplicantwillgetacomputer
generatedslipindicatingthedateastowhentheyshouldcontacttheconcernedofficeforgettingrationcard.
5.TherationcardcanbecollectedbytheapplicantfromtheofficeofDistrictFoodandSuppliesController/
AssistantFoodandSuppliesOfficer/InspectorFoodandSuppliesORconcernedTehsil/DistrictLevel.
FeeDetails:Rs.10/(NoFeewillbechargedforBPLFamilies)
