Program Goals

Year One Goals
Define and describe the term 'self-regulating' and what it means to a part of a 'self-regulating' profession. Build a sense of identity
between building a sense of self and profession.

Met with and talked with my resident for at


least half an hour, each week. Learned a lot of
personal information about the clients past,
successes, family, and learned how she feels
about her health.
Started a My Day poster on resident which
includes information one would not know
without having a relationship with the client,
and knowing her well.
Daijauna is learning how to build and
maintain therapeutic relationships with the
residents. She spends time conversing with
and trying to understand their views.
Established a therapeutic relationship with
another resident in the LTC facility. Ensure to
go speak with her for a while every week, and
let her talk to me about her life.
Continually conversed with multiple residents
in LTC facility learning both verbal and nonverbal techniques on how to communicate
effectively.
Completed a My Day poster for my
resident.
Learned that many residents have a routine
they like to follow after breakfast. Some like
to sit by the window and listen to music,
while others like to watch TV in the common
area or in their rooms. I also learned that one

Helped multiple residents with their morning
care. For residents that are only partially
compensatory, I would situate them in the
bathroom after helping them wake up and get
dressed to allow them to complete care on
their own (situating by the sink so they can
brush teeth, wash face, etc.). With some
residents I would brush their hair for them.
Daijauna is learning skills that encourage the
residents to participate in the activities that are
provided by the LTC facility. This way she is
encouraging social interaction with the
residents. She also spends time conversing
with the residents therefore allowing for
social interaction.
Spend time with multiple residents each day
to ensure they have some kind of social
interaction, as it can be hard for the other
workers to have the time to carry a
conversation.
Convinced some residents to join in on the
shuffleboard game that occurred before the
outbreak when activities were cancelled. They
were very happy to get out of their rooms and
had a lot of fun.
After my resident suffered a hip fracture, she
needed a lot more assistance with walking. I
have helped her walk back and forth from the
dining room and washroom.
While changing multiple briefs, helping to get x
residents dressed and undressed, and bathing
residents always completing a basic skin

assessment to see if there is anything out of


the ordinary (rashes, ulcers, marks, etc.).
While feeding residents in the dining room,
always keeping an eye on whether they are
swallowing with difficulty, or choking. Also
assessing how much they eat to compare it to
their normal eating habits.
Daijauna is learning and developing basic
nursing and assessment skills. She is able to
measure a BP on another student as well as
participated in a mock head to toe assessment
on a student.
Completed a head-to-toe assessment on peers
as well as my resident including metal,
respiratory, abdominal, and circulatory, etc.
Used the Morse Fall Scale to assess my
residents risk for falls.
Assessing the environment in my residents
room when I walk in to ensure there is no
obstacles or things to increase her risk for a
fall.
Encouraging resident to go on a walk to
stretch her legs and move around a little (has
arthritis) instead of sitting in her room alone.
Resident doesnt socialize often, so
encouraging her to leave her room more than
just for breakfast, lunch, and supper is
beneficial for her.
Encouraging resident to help as much as
possible with morning care. Helping the
resident get dressed and out of bed, but
encouraging the resident to complete the less

Daijauna is learning how to provide client
centred support for completion of ADLs and
bathing of residents. She is also able to
complete brief changes and other support
activities.
My resident is fearful to leave her room on
her own ever since her recent incident. I
ensure she feels comfortable before helping
her walk to the dining room. I ensure she has
everything she needs close by her chair before
I leave her room, as she cannot get up on her
own right now.
Used a lift and bath chair to transfer residents
into the tub, and completed multiple full baths
including peri-care.
I have learned that some residents are very
particular in how their beds are made, or in
how they would like to be dressed. I have
adjusted my skills for them when necessary to
insure Im providing client-centred care.
Learned about how much dignity is lost by
each resident since they cant do what they
used to anymore. They have no privacy and
must be horrified when they have to go from
doing everything on their own to having
people feed, bathe, and move them.
Learned about the many activities that occur
every day in long-term care including
shuffleboard, cards, darts, exercises, etc.
Learned about how these are important
because many of those in long-term care are

depressed or socially isolate themselves.


These activities help to encourage residents to
leave their rooms and create some
relationships to increase their quality of life.
Daijauna is beginning to understand how the
residents might feel about living in a LTC
facility by discussing it with them. As she gets
to know them she understands their
preferences
My resident is now scared to walk around in
her own home ever since a resident pushed
her and broke her hip. Reflected on how
horrible it must be to not feel safe in your own
home.
Learned that all residents have a very different
view on long-term care. Some residents are
very depressed or upset to be there while
others love it.
Learned how important some of the activities
the facilities provide are to residents. Its
something to keep them busy, and entertained.
Many were very disappointed when they were
cancelled due to an outbreak.

Safely completed multiple lifts and transfers
with a PSW (full lifts, sit to stand, etc.).
Practicing blood pressure and all other skills
in labs and on peers before on residents in the
facility to ensure correct safety for the
residents.
Learned techniques from the PSWs on how
to preserve clients dignity and keep as much
privacy as possible while completing baths,

competent practice that is appropriate for a

Completed a head-to-toe assessment on my
resident, including vitals.
Followed proper PPE droplet and contact
precautions when our facility was in outbreak
for influenza B.
Always performing proper hand hygiene.
Ensuring all safety precautions are followed:
Call bell accessible to each resident, safety
belts used appropriately, alarm and yellow
banner across doors when necessary, etc.
Completed a reflection each week on an event x
that occurred. Reflected on things I couldve
done better and things Ive done well. One
week I reflected on how I need to ask the
PSW before helping a resident I dont know
because even though I knew the resident was
able to get up and dressed on her own, I didnt
know she was aggressive. I helped her and she
got very agitated
Always asking PSW or clinical instructor
when unsure of something for assistant and to
ensure I am completing a task properly and
safely.

In my therapeutic encounter I reflected back
on how I built a therapeutic relationship with
my resident. What I did well and what I could
have done better.
Completed my final reflection looking back at
my entire time at this placement in the longterm care facility.

Overcame misconceptions about dementia
learning that there are various types and not
all residents with dementia are unable to
communicate properly. It doesnt affect their
ADLs all the time. Also, not everyone in
long-term care have dementia.
Overcame misconception that all residents in
long-term care were wholly compensatory.
Some residents are able to do a lot of things
for themselves.
Daijauna is learning about her personal beliefs
and attitudes regarding the elderly and caring
for them
She does not allow her own personal biases to
interfere in the provision of safe ethical care
Learning that my personal values may be
different from those of others and that I cant
let them come between the care I provide.
Always reflecting back on my personal
attitudes and values to analyze what I need to
change and control.

Discovered how there is a great difference in
how people age.
Through weekly care cards learned about
multiple chronic illnesses including GERD,
arthritis, anxiety, diabetes, etc. Learned many
nursing interventions to help with cope with
these chronic illnesses.
Developed a better understanding of ulcers
from first hand experience, seeing a stage 4
ulcer while changing a brief.

discussions around chronic diseases, the
medications and the possible interventions
Completed more care cards including
depression, hip fracture, stroke, etc. Learned
more nursing interventions, as well as many
from listening to others present their care
cards.
In my therapeutic communication I made a
nursing diagnosis and looked up interventions
for residents with impaired mobility due to a
fractured hip.
Developed an understanding of the
importance of small nursing interventions,
and how beneficial they can be. For example,
one resident was refusing a bath and as soon
as I provided her with information about why
it is important and educated the resident she
cooperated and agreed to take the bath. This is
also an example of person-centred care

because educating the resident is critical.

Identify 3 personal strengths developed in this placement.


1. Establishing good therapeutic relationships with multiple residents. I have been able to build two good therapeutic
relationships involving trust, respect, and mutuality. Having a very caring attitude greatly aided with this relationship
building.
2. I am becoming very good with reflective practice. I always think back about what could have been done differently, and try
to improve my skills for the future. This will be critical for the rest of my nursing career to ensure I provide the best and
most ethical care.
3. One last strength of mine includes that I have become very good with my time management skills. I have learned from
PSWs how hard it is to get so many residents ready in the morning, and different ways they overcome the stress and get
things done on time. I have transferred these skills to manage my time better for different assignments and projects outside
of clinical as well.
Identify 3 areas requiring further development.
1. It is important that I learn to have more confidence in my judgments so that I can take charge, and not have to ask as many
questions to make sure Im doing things correctly and ethically. Residents seem to answer better and cooperate more when
you seem confident with your presence and they can trust you know what youre doing.
2. It is important that I start to learn and understand my values and recognize when I need to put them aside to provide proper
care. I want to be able to provide the best care possible no matter how much I disagree with residents values, beliefs, and
behaviors, as it will be important throughout my nursing career.
3. I would like to learn some proper body mechanics to complete lifts and help transferring residents. This will be beneficial
throughout my nursing career so that I do not injure a resident or myself.
