Introduction including overview of area inspected and activities taking


place:This inspection is carried out at "ROYAL CENTRINO" a construction site located in Mumbra,
Maharashtra state, India on 02.07.2013. The site is famous for its construction of 14 story tower of
4 building considering of 3 wings A, B, C.
The total site is spread over 14 acres of land. Building 1A, B, C is completed and finishing work is
going on like painting, plumber, lift system etc. Building 2A, B, C is under construction and
excavation is started for building 3A, B, C.
Some of the areas inspected are:

Machinery used in construction.


Site security and pedestrain.
Excavation area.
Welfare facilities.
Work station office.
Workers health and safety.
PPE'S used by workers

The basic risk and hazardous activities that are found are work with machinery, manual handling,
waste handling, excavation, scaffolds, painting, cutting, DSE work station, welfare activities etc.

Executive Summary:Organization has to focus on five main areas highlighted in this report to improve their health and safety
performances. These are listed below in the order of priorities and importance. Detailed recommendations
with timescale and approximate cost are explained in the recommendations of this report.
1. Pedestrian safety.
Collision Risk of supply chain vehicle with pedestrians should be reduced by providing separate routes for
both vehicles and pedestrians with appropriate lightings, humps on the road, and speed restrictions within
the premises.
2. Machinery maintenance and monitoring.
Machinery maintenance should be carried out as per planned preventative Maintenance schedules and the
Entanglement and cutt-off hazards associated with the machineries should be identified and eliminated
through the developments of safe system of works, periodic safety inspections and safety tours by the site
supervisor.
3. Violence at work.
Risk of violence's at workplace should be eliminated through the proper execution of disciplinary
procedures, introduction of SAP system for proper documentations, prohibiting double shift work patterns
of the organization.
4. Contractor Management.
Selection of the contractor should be done through proper verification of the contractors Health and Safety
policies, accident histories, past performances, Resources and infrastructural capabilities, enforcement
actions, previous audit reports etc. Excavations made by the contractor should be secured and sealed
immediately to prevent the fall off objects and fall off peoples. Appointed Site Manager should PlanCoordinate-Monitor the contractors work on regular basis to ensure the organizations safety compliances.
5. Fire safety.
Risk of fire at work should be eliminated through providing proper fire extinguisher in cutting area.
Finance required to purchase fire extinguisher is too less than the loses due to fire. It will prevent worker
life and damage to material.

These improvement plans specified in this report will reduce the risk of accidents and ill-health. It will help
the organization to prevent enforcement fines and penalties, reduce equipment and property damage, sick
pay, absenteeism, cost associated with investigation etc.

Main findings of the inspection:


1. Non segregated pedestrian pathways and supply chain vehicle pathways elevates the risk level of
collision of vehicle with pedestrians. Movements of people during heavy vehicle movements is
dangerous.
These represent possible breach of Occupational Health and Safety Act amended 16 June 1999 RTI
2007,3,11-entered into force 01.03.2007(44) Workstations, traffic routes and other areas or
installations outdoors which are used or occupied by the employees in the course of their activity
must be organized in such a way that pedestrians and vehicles can circulate safely.
2. Many machinery operation are under risk due to lack of supervision, poor inspection and
ineffective Planed Preventative Maintenance. Coupling unguarded of concrete mixer machine can
result in a dangerous situation and lead to entanglement and cut-off of hand of workers.

3. Visible and sample evidence about the risk of violences are found in many areas of construction
because site supervisor and worker are working 12 hours shift. Risk of violence is high amongst
worker as they are doing hard activities for long time.
These represent possible breach of International Labor Organization convention No.189,
Recommendation No.201, Domestic workers Recommendation, Members, in consultation with the
most representative organizations of employers and workers and, where they exist, with
organizations representative of domestic workers and those representative of employers of
domestic workers, should take measures, such as to: Provide an adequate and appropriate system
of inspection, consistent with Article 17 of the convention, and adequate penalties for violation of
occupational safety and health laws and regulations.

4. Competences of approved contractor did not comply with the health and safety standards as they
did not demonstrate any visible commitment towards health and safety. Excavations made by an
external contractor was not sealed and secured and it leads to the risk of fall of people, fall of
objects etc.

These represents possible breach of ILO Occupational Safety and Health 2001 guidelines on
occupational safety and health management system clause 3.10.5.2 (a) which states that
arrangements for contractors working on site should include OSH criteria in procedures for
evaluating and selecting contractors.

5. There are ignition sources like wood, ply, paints etc and cutting of steel is observed with no fire
extinguisher present in that area. As arc is eliminated while cutting can result in huge fire which
will tend in huge loss of material and lives.

This represents possible breach of ILO C-155 Article 16-2 states that Employer shall be required to
ensure that so far as reasonably practicable, the chemical physical and biological substances and
agents under their control without risk to health. Article 18 Employer shall be required to provide,
where necessary, for measure to deal with emergencies and accidents, including first-aid
arrangements without risk to health when the appropriate measure of protection are taken.

Conclusions:The construction activity, office premises and welfare facilities is fairly well running, the
management demonstrates some visible examples of good health and safety practices at
workplace. Some significant hazards are identified in this report which relates to machinery
hazards that can cause entanglement and cut-off and electrical shock due to lack of supervision
and poor planed preventative maintenance. Risk of violence due to poor work pattern and culture,
collision of vehicle with people and improper selection of contractor. Action are recommended to
be taken include the development of planed preventative maintenance, inspection, review of
policies, development of safe system of work, development of permit to work, monitoring and
supervision. Develop violence prevention programs over working, conduct contractor audit. This
recommendation does not require any financial investments. Pedestrian pathway, machine guard,
and fire extinguisher require little finance which may prevent future losses in term of direct and
cost associated with accident.
The report has identified several legal breaches of health and safety low, which, if left could lead to
act by the enforcing authority. Many international occupational health and safety statutory and
regulate bodies demand safe place of work, safe environment and equipments, safe system of work,
training and supervisions and competent staffs to demonstrate a visible commitment by the
employer towards health and safety. The recommendations in this report will help the organization
to meet their legal obligations.
Recommendations in this report will help the organization to prevent accidents which will help
them prevent the direct and indirect losses. Direct costs are claimable, but indirect costs, which are
not claimable, are generally 35% more than the direct costs. Management actions towards this
report will improve the morale peoples associated with the company and it will help the
organization to improve their business performances.
