1. Demonstrate and properly perform vital signs and head to toe assessments.
Activities, Necessary Resources and Timeframes to achieve my goal?
- Practice preforming vital signs; pulse, blood pressure, respiratory rate, O2
saturation and temperature, on peers, residents and staff (weekly)
- Attend lab and complete quizzes on various vital signs (weekly)
- Preform head to toe assessments on peers, residents, and staff; noting level of
consciousness, balance and gait, skin color, nail color and cap refill, respiratory
rate, edema (weekly)
Evaluation
How will I know that I have achieved my goal?
- Practiced vital signs and head to toe assessment in every lab and clinical session
- Confidently asked by staff to preform daily vital signs
- Compare vital signs to base line record to ensure accuracy

- To provide safe and ethical care to residents in LTC.

- Knowledge and constant proficiency in hand hygiene techniques (weekly)
- Attend lift training, and properly use lifts with the assistance of a staff member
(weekly)
- Engage in compassionate care through speech and actions to understand the needs
of the resident (weekly)
- Question and intervene to address unsafe or unethical practices (weekly)
- Maintain good health, and keep up to date on current immunizations (by first
clinical placement)

- Constantly and effectively wash hands when needed
- Efficiently use lift with staff assistance
- Inquire as to resident wants, need or habits
- Evaluate situations and report to clinical supervisor or charge nurse if a situation
is unethical or unsafe
- Received all immunizations, and handed in all NAR
