Clinical Instructor Comments (Any area marked unsatisfactory need to be commented on).
Mackenzie completes her assignments in a timely manner each and every week. Mackenzie comes to
each clinical day dressed in a professional manner with a clean uniform, hair tied back and her name
badge presented at eye level. Mackenzie develops and maintains therapeutic relationships by
respecting each residents home (ie knocking before entering, cleaning up after completing care and
ensuring the residents personal belongings are in the preferred spot), spending extra time with the
residents by gathering an understanding of their life and past health history, and ensuring their privacy
(ie closing doors). Mackenzie will continue to build and maintain her therapeutic relationship each
week by spending that extra time with the residents. To further enhance her practice, Mackenzie should
remember to preplan her actions and having all supplies readily available to increase the efficiency of
care while maintaining a high standard. Mackenzie understands that each resident has a different and
unique experience living in a long term care facility (ie. Enjoying some activities rather than others,
food preferences etc). Mackenzie ensures she reviews each individual's plan of care and builds a
partnership with the residents by enabling the residents to guide their care (ie. offering choices, asking
questions etc.). For residents that are more dependent Mackenzie needs to always communicate with
the resident prior to and while assisting with care. Mackenzie needs to ensure she introduces herself
each week with her designation as many residents have memory deficets and the residents also see
many student visitors. Mackenzie ensures a safe environment for both herself and the resident each
time she assists with care. Mackenzie always raises/lowers the bed, follows each individual care plan
and seeks assistance when necessary from the clinical instructor or staff. Mackenzies knowledge, skills
and ability will continue to evolve each week if she seeks new opportunities and steps outside her
comfort zone. Keep up the Great Work!
