AFFIDAVIT FOR OUT OF TOWN REGISTRATION OF BIRTH
I, ___________________, of legal age, Filipino, married, and a resident of
Barangay Lakit-Lakit, Bongao, Tawi-Tawi, after having been duly sworn to an
oath in accordance with law, do hereby depose and state:
1. That I am the cousin of _____________who was born on __________ at
Poblacion, Jolo Sulu;
2. That the fact of his birth was not reported or registered before the
Municipal Local Civil Registry of Jolo, Sulu where he was born due to
the fact the he and his immediate family already transferred their
residence from Sulu to Lakit-Lakit, Bongao, Tawi-Tawi. His fact of
birth was not also registered in local civil registry of any other place;
3. That I execute this affidavit in order to attest the veracity of the
foregoing narration of facts, to support the registration of the fact of
birth of the said ___________________ before the Municipal Local Civil
Registry of Jolo, Sulu through the Municipal Local Civil Registry of
Bongao, Tawi-Tawi, and for whatever legal purposes and intents that
this affidavit may serve best;
