Was the candidate in a position to supervise others? If so, how would you describe their
management style?

While Haven was not exactly a supervisor she did help to orient new PCTs. She
would help show them their jobs and tasks on the unit. She also helped to reorient
PCTs who needed extra reinforcement. Haven did an excellent job in this type of
role. Everyone who has been oriented by Haven has become a strong team player
on the SIMC.
How would you describe the overall quality of the candidates work? Please give
examples.

Just for starters, when I see Havens name next to mine on the assignment board I
know it is going to be a good day no matter what kind of challenges may lie ahead.
Haven goes up and beyond to do her job well. She gets all of her work done in a

thorough and timely manner. She also coordinates with other nurses so that the day
can run as smoothly as possible. When asked to do something Haven ALWAYS gets
her work done within a reasonable time. One time I had a very critical patient who
needed multiple travels, blood transfusions, and much more. Haven was the only
tech on the floor that day. She assisted me during all the travels ran to blood bank
and got all my units for me AND she still managed to give the other patients on the
floor optimal care.
Haven is a huge patient advocate and always has a positive attitude. She
always performs preventative interventions for patients including turning, and
hygiene. Haven is always prompt in responding to callbells. We recently as a unit
had low scores in this area. As these scores go up I can assure that Haven was one
of the key players in this accomplishment.
Another day that really sticks out is a time when Haven reported something
abnormal to an RN that ended up bringing the patient code one to the OR. Because
of Havens thoroughness on a daily basis many complications happening to our
patients have been prevented.
Haven is an excellent fall preventer as well. One time half of the floor had
callbells and bed alarms going off every five minutes. Between the diligence of the
RNs and Haven we managed to go that day fall free!
How did the candidate handle pressure and/or deadlines?

Haven is excellent at handling pressure and deadlines. She is always timely with her
assignments and is able to adjust well when workflow changes. Even when the day
becomes absolutely hectic Haven still keeps up with the day AND manages to
maintain a positive attitude.
How would you describe the candidates ability to communicate with others?

Haven is excellent at communicating with others. She starts her day with
coordinating plans for patients with the RN. She understands the abnormals to
report to the nurse and does so in a very timely and efficient manner.
How would you describe his/her relationships with coworkers, subordinates (if
applicable), and with superiors?

Everyone on the unit has seen Haven as an extremely devoted and hard worker.
Haven meets the highest standards in her role and when orienting new PCTs we
often will use her as the example to follow.
Why did he/she leave this position?

Haven is in her last semester of nursing school and will be assuming the role of an
RN once she passes her NCLEX.
Were there any issues we should be aware of before making a hiring decision? (ie.
Attendance, punctuality)

Haven was always on time, not just physically with clocking in but also mentally
prepared for the day. The only times that Haven has called out is for illness reasons
which rarely happened. She is a gold star employee and had no issues of concern.
Would this individual be eligible for rehire? Why or why not?

The candidate is being considered for a position as a registered nurse with UMMC. Based
on the candidates performance with your company, do you think he/she would be a good
fit for the position were considering them for?

100%. She is a devoted employee and an excellent critical thinker. She is also very
intelligent when it comes to nursing. In addition to this she is very caring and
compassionate and shows both sympathy and empathy toward patients.
