environment. For example, i practiced hand hygiene every time i go into the room of the patient. I make sure i sanitize my hands and wear
gloves. After providing care for the patient, i take off the gloves and sanitize my hands again to prevent spread of infections.

Oladapo adhered to regulatory standards by practicing according the the CNO Standards of Practice and according to the policies and
procedures of the clinical agency. For example, when administering IV medications he followed the unit policy by referring to the IV
manual. He has demonstrated the ability to provide effective nursing care with minimal assistance. As well, she continuously evaluated
areas for improvement and sought assistance when necessary. He demonstrated accountability by ensuring that he provides the best
possible care for his patients. For instance, he demonstrated the three factor framework by ensuring that his patents were safe and
comfortable at all times, and by ensuring that the environment was free of clutter with had proper lighting. Oladapo was aware of his own
values and ensured that they did not conflict with his practice.

Over the course of the semester Oladapo learnt to communicate effectively with his patients and with the inter-professional team. Initially,
he talked over the patients with no eye contact, but by midterm he was stooping down to the patient's level and maintained eye contact
during conversations. He ensured that he received a report of his patient's status every morning and learnt to explain to the patients the
care he was about to provide prior to initiating the actual care. Initially, he struggled with documentation but by week 6 he was
documenting accurately and concisely and in a timely manner. As well, he assisted his peers to understand and provide documentation on
their patient's care. Good job!

O.O.J. : I always advocate for the client, myself and peers. I always advocate for quality care for the patient. For example, I assessed my
patient and she told me she is in a lot of pain. I went straight to the primary nurse of my patient to inform her. She didn't want to give her
the PRN pain medication. She wanted to wait for her scheduled medication that will be due in 2 hours time. I advocated for my client and
the nurse agreed to give her the PRN pain medication immediately.

Oladapo demonstrated leadership skills by assisting his peers to provide competent patient care. For example, in week 9 he was able to
assist his peers to administer medication by observing and guiding them to perform the 3 checks and 8 rights. As well, he collaborated
with his patients, staff and instructor to provide respectful professional nursing care. He demonstrated an eagerness to learn by voicing a
desire to learn certain skills when available. He was very organized and was instrumental in assisting some of his peers to organize their
care.

example, one of my patient was having difficulty breathing, i will raised the head of the bed and put it in high fowler position and i

intervention implemented will be administering anticoagulant drug and administering oxygen to enhance ventilation.

supervising me. For example, when i get to the clinical setting, i plan my day and write down on the organizational sheet what i will be
doing every hour. I write down the time i will need to administer medication or perform a procedure.

Oladapo exhibits the knowledge, skills and judgement necessary to provide safe competent nursing care. He utilized the three factor
framework to guide his practice by ensuring that his patients were safe at all times and that the patients' environment was safe by ensuring
adequate lighting, making sure that the rooms were free from clutter and making sure that his patients were comfortable. Over the course
of the 14 weeks he learnt how to prioritize and organize his patients' care by utilizing an organizational plan and by performing the most
important task first. For example, administering pain medication before providing a bed bath. As well, he prepared two concept mappings
satisfactorily to include interventions necessary to provide optimal health for his patients. Good Job!

meet the needs and provide the best care for my patient. For example, There was a time i was doing the abdominal assessment of my
patient when the doctor came in to check on my patient, i just finished auscultating for the bowel sounds of my patient. Just to confirm
what i heard was correct, i asked the doctor if he could check for the bowel sound of the patient. The doctor checked for the bowel sound
of the patient and we both agreed there was no bowel sound.

teacher into my practice. For example, When my partner had five IV medication they will be administering, i spoke to my partner if i could
administer two of the IV medications. We spoke to the clinical teacher about it and i was allowed to administer the medication.

in the hospital where i do my clinical now. Now i am using meditech to do documentation in the hospital

Oladapo critical thinking skills have increased over the course of the semester as he has demonstrated an increased depth of

I learned a lot this semester. I am very confident in administering primary and secondary IV medication, oral medications, subcutaneous
injections,precise and accurate documentation, applying my theoretical knowledge in clinical practice. I am still developing my critical thinking,
problem solving and decision making skills to meet the health care needs of my patient. The strategies for my continued growth include using
research to foster professional development, seek out more learning opportunities and feedback, using theory-based approach and evidenceinformed practice and practicing self reflection.

Oladapo is very accountable, responsible, respectful and professional. He maintained attendance over the full 14 weeks of clinical and was always
punctual and well dressed in full uniform with his name badge visible to identify him as a student. Oladapo exhibits a wealth of knowledge in nursing
concepts and endeavored to utilize his skills to guide his practice. Oladapo has successfully met most of the goals and objectives that he planned to
achieve over the course of the semester. For example, he was able to administer oral and subcutaneous medications independently by week 14, and
was able to administer IV medications with minimal assistance from his instructor. Oladapo developed confidence in IV administration, including
hanging a secondary line, priming IV lines, preparing and mixing the medication after having his instructor double checking to ensure that he had the
right medication for the right patient at the right time. His documentation skills have improved as well as his overall patient care abilities. Overall,
Oladapo demonstrated the ability to function competently in all aspect of patient care listed under his scope of practice. He was informed that his
critical thinking skills, problem solving and decision making skills will develop overtime as he continues to review and watch the nursing videos
provided for him by the college.
