Ralph demonstrated accountability and responsibility each clinical/lab day.I agree that there were times when I thought he was capable of
much better performance. He has upheld both Humber's student handbook guidelines as well as the policies and procedures at
Etobicoke General Hospital 9E/9W LTC/Reactivation. His conduct demonstrate respect and professionalism and abided within the
ethical/legal framework. He maintains privacy, confidentiality and promoted safe environment by taking appropriate actions such as
ensuring brakes on bed and other equipment, bed rails, bed in lowest position and alarms on the bed with his client with Dementia. He
fully understands the limitations of his role and seeks necessary guidance when he is unsure.

documentation lab on the very first day of clinical was very helpful as I was able to navigate and document very easily through the system
the hospital used; this allowed me to document fast and help others to document when they needed help.

understanding; for example, I had a client that also spoke my native language. Because of this, it was easier for me to identify what my
patient wanted and I was able to translate my patient's words into English where the nurse in charge could understand.

Ralph learnt the Medi-Tech system very fast at the beginning of the semester. He was very good at retrieving patient data as well as
daily documentation of ADL flowsheet, Vital Signs, Mental/Emotional Status, Braden Scale, Wound Care, Catheter Care and Narrative
Nurses Notes. He was very supportive with his peers in learning this electronic documentation. He used effective communication
strategies within the interdisciplinary and among his peers in the clinical setting. He shared relevant information regarding his new
learning with his peers in post conference. He fully understands importance of maintaining confidentiality and privacy.

Ralph developed therapeutic nurse-client relationship with her clients and their families and professional relationship with health care
team members on the unit. As he cared for clients from diverse cultures, he demonstrated cultural sensitivity. He had very good
interactions with client. I agree that he assisted the staff in understanding the needs of a client who spoke his language.

management within the clinical setting is crucial in order to give proper care to each patient one is assigned to, I have been lacking this
skill from the start through midway of the semester; but, I believe I have improved by the end of the semester and am able to do such tasks
in a timely manner.

Ralph had good level of preparedness each clinical day. Medication and Diagnoses Research was adequate. His initiative, timemanagement, organizational skills and prioritization skills has improved throughout the semester. Ralph performs vitals signs including
SPO2 and Pain assessment. Other nursing assessments are improving and need to be practised on a daily basis. Nursing Interventions
that he completed included complete bed bath, transfers with assistance, wound care, catheter care. He safely administered medications
administrations orally and subcutaneously.

way to intervene this problem was I have contacted the nurse in charge on how to set and turn off the bed alarm to alarm anyone if my
patient is trying to get out of bed.

Ralph attended Patient Rounds and In-services on Stroke, the Role of the Recreational Therapist and Feeding a Patient with Stroke( by the
Speech Language Pathologis)t. He organized his client's care each clinical day to ensure that they were ready for their therapy.He fully
understands and respects the role of each interdisciplinary team member in the rehabilitative care of the client. He interacted well within
the interdisciplinary team.

Student Final Summary with Strategies for Continued Growth


There were many things that I've learned throughout the semester, one of those things are my weaknesses and aspects of my nursing care that I
need to work on. The aspects that I need to work on are to critically think in situations where I'm under pressure and be able to carry these well
thought out plans, to be able to manage time wisely and effectively, and lastly in general to improve every aspect of my nursing practice as there's
always room for practice and improvement.

Ralph has successfully completed the requirements of Semester 2 nursing clinical practice. He has showed interest in his learning and had a good
