M.T.H. : Privacy and confidentiality are two important things that client's want to ensure they are getting when a nurse is caring for them. I
am satisfactory in this because I made sure I reassure my patients ( classmates and stimulating dolls) and provide privacy at all time when

separate the person from the problem and discuss the issues with the person.

with it. I would try to separate the person from the problem and attack the problem and not the person.

practice in the open access lab. I also accepted feedbacks from peers which helped me during my final testing.

M.T.H. : As a nurse it is very important to plan care that is entered around the client because they are the most important factor in the
nursing process. Assessment should be conducted according to the symptoms that the client presented. It is also important to re-evaluate
all interventions to see whether or not the patient had improved.

M.T.H. : Interview is done by gathering subjective objective data from the patient. Vitals are taking and assessment done according to

M.T.H. : We did all of the above in class but I was only tested on musculoskeletal. I am well aware of how to conduct all of the
assessments. Vital signs include pulse rate, respiration rate, temperature, blood pressure, oxygen saturation and pain. Neurological
assessment includes the twelve pairs of cranial nerves,assessing the sensory and motor senses and conducting reflexes.

M.T.H. : I was not tested on the above activities but they were discussed in our class. I have the knowledge of how to conduct each. It is
important to wash least contaminated areas when providing hygienic care. This prevent the spread of infection. It is also important to
include client in their care by asking their preferences.

M.T.H. : I practiced this by washing my hands before and after caring for a patient. I also make sure I wear protective gear such as gloves. I

also tried to do all my readings pertaining to what would be discussed in class. For example when doing the cardiovascular I read my
textbook to locate some of the major areas of the heart, watched video to get an understanding of the assessment and referred to my
anatomy notes.

M.T.H. : This was done successfully throughout practice and assignments. For example on the abdominal checklist the order of
assessment changed from inspection,palpating, percussion and auscultation to inspection, auscultation, percussion and palpating.
Percussion and palpating we're done last because they can alter bowel sounds upon auscultation.

After twelve weeks of ongoing practice I have learnt al lot. These includes taking vitals, conducting interviews, assessing clients and providing care.
My plan for next semester is to build on my communication skills by asking questions in class and discussing openly with classmates. I would also like
to build on my interview skills by doing research.

I encourage Monique to work on her class participation and being able to ask questions during a class discussion. Monique is very professional and is
always in uniform and submits all assignments on time. Monique takes this class very seriously and I notice this on a weekly basis. I encourage
Monique to try to take on more of a leader role when it is necessary. Please have confidence in the skills that you know well and are performing. I
enjoy watching Monique work one on one with her peers and how receptive she is to their recommendations. I encourage Monique to put into place
some personal learning goals and to achieve them, as they will aid her in being successful in the nursing program.
