Either using the Systems Assessment or the head to toe physical
assessment.
I learned that thorough and accurate Assessment of a client is a
crucial and significant step of the Nursing Process as it will
greatly affect the rest of the methodology.
I rate myself Excellent when it comes to performing Physical
Assessment on a client/patient. I have achieved this proficiency
through a good grasp of textbook reading and learning, with the
help of the audio/video presentations that were provided, the
actual practice on scenarios during laboratories, and the mastery
of the procedure during Clinical Integration Assessment.
Learning how to Assess a client thoroughly and accurately was
significant because through this, the proper and appropriate
intervention and nursing care can be delivered and provided to
the client with emphasis on the priority needs and demands.
This Learning Experience of demonstrating the ability to
perform physical assessment will be very useful in my Nursing
Practice because it will enable me to identify and categorize the
immediate needs of a client and thus therefore provide the
nursing care that is required for a client's survival and/or care.
Assessment is crucial as this is the first step of the Nursing
Process, hence, the rest of the process greatly depends on how
precise and comprehensive the data collected.

V- Infusion therapy 5-Blood and Blood Products


V-5-9 Demonstrate knowledge and ability to manage a reaction to
blood or blood products according to agency policy.

My significant learning experience was during my guided lab practice


and CIA where I have actually handled a situation such as transfusion
reaction. One of our bed rotations was about blood transfusion and our
instructor clearly and thoroughly taught us about how to manage such
transfusion complication. I learned the importance of close monitoring
of a patient who is undergoing blood/blood product transfusion. The
first 15 minutes of transfusion is intentionally set on a slow rate (and
later gradually increase the rate if no reaction noted) to thoroughly
assess the patient for any reactions such as fever, hives or skin flushing,
back pain, chills, dizziness, and bloody urine. If a reaction is suspected,
discontinue the transfusion and keep vein open with a normal saline
solution with a new IV tubing. Compare the base line vitals signs with
the assessments done, and notify the physician immediately. Notify the
blood bank as well and send the blood bag and tubing for repeat typing
and culture. Document properly and accurately as to what has
chronologically occurred. Most agency policies follow this standard
procedure on how to manage a transfusion reaction.

I have learned that as a student nurse, I have to be alert and equipped


with skills and knowledge in identifying blood transfusion reaction.
Thorough assessment of the patient is always a good tool to avoid and
prevent further complication.
Thus, an accurate and effective
management of a reaction is provided and the patient is given the due
care needed to proper treatment and fast recovery. This was meaningful
because to have these skills, knowledge and attitude towards situations
like this, could save the patient from danger or worse yet even death.
Time is crucial in this kind of setting, therefore, as a student, I learned
that I should be able to identify the problem in a timely manner and
already think of providing the nursing management to the patient
according to agency policy.

I would rate myself good as to demonstrating the ability to manage a


reaction to blood/blood product transfusion. I have correctly and
accurately answered my instructor's questions during the guided lab
practice and I have successfully demonstrated my skills on how to
manage a transfusion reaction with our blood transfusion kits and
pretend patients during the lab. I did a lot of reading of textbooks and
articles online, watching videos, and practicing with my kits at home,
which greatly helped me during my labs, especially during my CIA.

This learning experience is very helpful to me as a student nurse and as


a future Practical Nurse because it has created a great impact towards
my motivation of becoming a good nurse and equipping myself with
the skills and knowledge of performing a thorough assessment of a

R-1 Community Health


R-1-7 Demonstrate knowledge and ability to provide health
teaching and coaching associated with community health
nursing.
One significant learning experience I have in this course was to
be able to complete a thorough Analysis Paper and constructing
a Teaching Resource on how to provide health teaching to a
focused group in my community. It is essential to follow the
community health nursing process. I have learned the
importance of assessment through proper identification of the
health education needs of the chosen target group, which in my
case, the high school students of my community at Westmount,
Edmonton. It is crucial that community participation and the
learning need is included in the planning phase of the health
teaching. The implementation phase is just as important as the
other stages, wherein a strategy is carefully executed for the
purpose of educating, reinforcing, correcting, and assisting the
community in the health teaching that has been identified and
thoroughly planned out. The stage of evaluation is the phase
where the community is appraised as to how much of the health
teaching has been grasped and applied in their daily living. The
focus of the whole activity is to instill and develop good habits
and practices, health promotion, disease prevention and harm
reduction in the target population of the community.
I have learned that as a student nurse, I have to be equipped
with the skills, knowledge and attitude in properly following the
nursing process in providing health education to the community.
The priority learning need of a target population in the
community must be carefully identified and the needs should be
addressed with the best strategies that are appropriate to the
chosen group. In my study, the health teaching need was
identified as The effects of tobacco smoking on high school
students of Westmount, Edmonton, as there was a high
prevalence of smoking teenagers as observed by the researcher.
As a student nurse, being able to formulate an Analysis paper
has given me an in depth experience of going through the
nursing process which has broadened my perspective and
understanding of my community. These high school students of
Westmount, Edmonton, are exposed to different populations of
the community especially that the school is located close to
public places such as the mall or the recreation area, where they

can easily see adult smokers that might trigger their curiosity
into smoking. It is important to give accurate information
about the effects of smoking to their health, especially the
physical and actual results, to discourage the youth to even start
and/or encourage cessation of such bad habit if ever they have
been smoking already. It is crucial that they are taught at an
early age for them to realize the long term effects of smoking
and the fact that it has been proven harder to quit the habit at a
later age.
I would rate myself good as to demonstrating knowledge and
