II.

Information Source
The tool used to gather this information was the checklist that was
provided by the instructors for the children ages 3-4 years old. The
checklist asks to observe all the different milestones from
communication skills to large motor to fine motor skills. I also sat
down with mom and asked several questions about J.Vs interaction
with other children and adults.

III.

Background Information
a. J.V is a 4 year old child that is living with both of his parents and
an older half-brother. J.V and his brother are our frequent early
morning arrivals since mom goes in early to work. Dad works in
construction so we hardly get to interact with dad. J.Vs
interaction with the children is from 6:30 in the morning to 4 in
the afternoon. This has helped him to develop many social skills
with the rest of the children and adults. When asked mom how
his interaction with others was at home she did mentioned that
he gets babied by most of the adults around him, so school has
helped him to become more independent.

b. J.V arrives daily early in the morning with a positive attitude and
a smile from ear to ear. J.V likes to come in and play with the rest
of the children with the Legos at the tables.
c. J.V enjoys always answering questions when asked. He is
frequently yelling out answers and is asked to please raise his
hand when it comes to answering a question or would like to say
something to the teachers. J.V always gets excited when it comes
to new activities and is always ready to try a new activity.
d. According to the assessment tools, J.V is naturally developing;
thanks to him having the opportunity of being more independent
he gets to expand his learning to all the areas around the
classroom. He has become more verbal and is a leader in the
classroom.

IV.

Developmental Performance
a. J.V has several areas of interest but the one that he spends more
time in has to be block areas. J.V will build new things around the
discovery area as well as likes to invite others to help him build
new things. J.V frequently likes to talk about his dad building
things and driving the huge work truck that sounds like a
monster every day.
b. J.V usually avoids the writing center; he has mentioned that his
hand hurts when he writes too too much. When it comes to
completing a small group activity J.V tries to avoid it. He is

constantly reminded to complete the work then he can move on

to the toy area.
c. J.V is a very affectionate child he enjoys giving hugs, holding the
teachers hand and sitting with the teacher. J.V is very attentive
about the other children. When a friend is hurt he usually asks
are you okay? Do you need an ice pack? He is a problem
solver, he asks his friends to use their words for example J.V
asked his friend Can you ask me nicely please to have a turn?

Conclusion and Recommendations

a. J.V is a very nice and respectful child he has demonstrated
growth amongst his classmates and adults in the classroom and
those who are not in the classroom. He is a problem solver and
likes to interact and play with the rest of his classmates; he
frequently is asking the children would you play with me?
b. I have made some accommodations for J.V when it comes to
work time. When I see that he has become frustrated and gets
up to wonder around the class. I ask him to take a two minute
break and then get back to completing the work. If I see that,
that did not help then I sit next to him and ask him would you
like for us to complete this together? he usually says No, let
me try one more time alone. Can I call you if I need help
teacher?
c. Some suggestions that mom and I came up with together was to
let J.V complete more task on his own. I encouraged mom to
have a talk with grandma to ask her to not do things for J.V, I

asked her to explain to her that it will help him to become more
independent and will help him in the future. I also asked mom
that if she would like for us to meet with grandma just to give her
an update on the growth of J.V since the beginning of the school
year and how school has helped him.
