Background Information:
Suzie is a hard worker who is also kind and helpful. She is able to stay on task during
class instruction and work times. Suzies teachers report that she is struggling with reading.
Suzie has been receiving reading support in a small group for the past year with Mrs.Reader ,
Learning Support Teacher. In third grade, she was taught using the Leveled Literacy Intervention
program of which she finished the blue level except for 5 lessons. In fourth grade, she has been
involved in the SRA Decoding program as well as working in the Fast For Word Language
program. Her fourth grade teacher is using repeated readings to help her improve her reading
fluency. Her parents also started hiring a private tutor to work with her twice weekly for one
hour. The tutor is using the Wilson Reading System with her.
Academic Evaluation:
Suzie was given the Weschler Individual Acheivement Test II. This is a test of basic
achievement in core academic areas. Suzie was cooperative throughout testing and tried items
even though some of them were difficult for her. Some of the components are timed to determine
how efficient and fluent basic skills are while other components are administered until the student
misses a certain number of responses. The subtests are scored based on Standard Scores. The
average range of scores is between the scores of 85 to 115. The subtests are combined to obtain

In a small testing situation Suzie demonstrated that she was able to follow oral directions
and express ideas in a timely fashion. In the reading components, Suzie performed significantly
below grade level. Her reading fluency is choppy due to her attempts to sound out unknown
words. She uses decoding strategies to sound out most words that she does not know, however,
she is unsuccessful in decoding many of the words. She struggles with decoding predictable as
well as unpredictable words. She does use context clues to self-correct some of the time when
she misreads a word, but typically she just keeps reading. She reads very loudly. Suzie is near
grade level in math. For written expression, she has great ideas and was able to write
grammatically correct sentences. She had great difficulty with spelling even with phonetically
regular words.

Suzie took the Measures of Academic Progress (MAP) tests for reading and math in fall

Classroom Observations:
Suzie is a cooperative student in the regular education setting. She has friends and is very
compliant. Suzie was observed in a large group reading instructional time. She followed along in
class, was on task 90% of the time and was very attentive during class. She worked well with her
group for the helping verbs partner activity. She kept her materials organized throughout the
lesson.
Summary/ Recommendations:

Suzie is a very nice young lady who works hard in school. She wants to improve her
reading skills. Suzie has not made reading gains over the past two years even though she has had
interventions in regular education. At this time, Suzie appears to be significantly below grade
level in reading and spelling. Her written language appears to be near grade level except for
spelling. Her math appears to be at grade level.
Goals:
Given remedial education Suzie will increase her reading MAP score one and half grade years.
Given remedial education Suzie will increase her spelling to 85% accuracy in 3 out of 4 trials.
