For NEW and RENEWAL applications of S2 license

Please bring original and photocopy of the following:

S2 LICENSE REQUIREMENTS
(1) Valid PRC ID Card please ensure that PRC IDs are renewed prior to applying for S2
license renewal

(2) Valid Professional Tax Receipt (Private practitioners) / Latest Certificate of

Employment and original duly-notarized affidavit that the S2 license shall be used
exclusively in government practice only (for government practitioners)

(3) Tax Identification Number (TIN) card or latest Income Tax Return (ITR) W-2 form filed
at BIR

(4) Valid drug test result prior to S2 license application (DOH-DDB IDTOMIS-generated
report)

(5) One (1) piece 1 x 1 ID picture with white background/no eye-glasses

One (1 ) piece 2 x 2 ID picture with white background/no-eyeglasses

(6) Previous original S2 ID issued (for renewal applicants). For lost S2 ID, submit proof of
publication of loss

(7) Self-addressed and stamped return (postal) envelope (actual plastic S2 license which
will be released 15 working days after application and processing)
Important:
a) S2 license fee is P1,500.00
b) Government medical practitioners exempted from paying S2 license fee
c) S2 validity shall be harmonized with current PRC validity
