This document contains two different forms of recognition. You are


welcome to print these pages as evidence of your academic
achievement; however, if you need official documentation of your
grades, please contact the Enrollment Center (503.399.5001) for
information on ordering official transcripts.

Academic Recognition
Winter Term 2014
Congratulations!
Your academic performance at Chemeketa Community College
during winter term 2014 has earned you this special form of
recognition.

This document includes:


On Page 2: A recognition letter which details your achievement and is
signed by the college president and the chief academic officer.
On Page 3: A certificate of recognition with a colored border. Printed
using a color printer, this will provide you with a permanent memento
of your recognition which is suitable for framing.

Academic recognition at Chemeketa comes in three different


forms:

On Page 4: A borderless certificate which you may print on certificate


paper (available at most office supply stores). Certificate paper has a
pre-printed border which occupies about 1.5 inches all the way around
the page. This version of your certificate should print in the center of
such paper.

Honor Roll recognizes students who earn a term grade point


average (GPA) of at least 3.00 while completing six or more
credit hours.

Chemeketa will maintain a copy of this academic recognition document


for at least one year after it is earned. We recommend that you save or
print the e-mail message that originally directed you to this document
in case you wish to download it again in the future.

The Dean's List recognizes students who earn a term grade


point average (GPA) between 3.50 and 3.99 while completing
twelve or more credit hours.
The President's List recognizes students who earn a perfect
4.00 grade point average (GPA) while completing twelve or
more credit hours.
Your term grade point average of 3.08 has earned you
placement on Honor Roll. (You have been placed in the highest
category of recognition for which you qualified this term.)
In addition to this recognition document, your name may have
been submitted to newspapers in Chemeketa's district (including
the student newspaper) for possible publication in printed
academic recognition lists.

About The Phi Theta Kappa Honor Society...


Phi Theta Kappa is an international honor society for community
college students. Chemeketa's chapter of Phi Theta Kappa follows the
national guidelines for the qualification, invitation, and induction of
Chemeketa students. If your academic history qualifies you to join Phi
Theta Kappa, you would have received a separate notification of this
eligibility. The criteria for membership in Phi Theta Kappa are different
from the criteria Chemeketa uses for academic recognition!

Life Department in Building 2 on the Salem Campus (telephone:
503.399.5116). Staff in that office will be able to give you information
about the society's current advisor.

We know that college is just one facet of your busy life. Chemeketa students work full- or parttime jobs, have families, are active in the community, and have numerous competing
