Assumption of Risk
I _________________________ freely choose to participate in Tiffin Calvert Catholic High
School Flag Football Fundraiser 2016 activities which include participating in a flag football
game. I know that injuries can occur during the event that can include, but are not limited to,
strains, sprains and trip/fall injuries. I understand that participation is voluntary and is not part of
my work related duties within my regular job. I have been advised to consult with a medical
doctor with a regard to my personal medical need. I agree to accept any and all health risks
involved with participating in the event.
By Signing this Assumption of Risk and Release of Liability Form, I affirm that there are no
health related reasons of problems that may preclude or restrict my participation in the event.
I also recognize that Calvert Catholic schools are not obligated to attend any of medical needs or
medication needs and I assume all risk and responsibility thereof. In case of a medical
emergency during the event.
I authorize in advance the representatives of Calvert Catholic to secure whatever treatment is
necessary. Calvert Catholic may (but is not obligated to) take any action it considers to be
warranted under the circumstances regarding my health and safety. Such action do not create a
special relationship between Calvert Catholic and me. I release Calvert Catholic, its officials,
staff, employees, volunteers, students and coaches from all liability for any bodily injury or
damage that I may sustain as a result of any medical care the I may receive resulting from
participating in the event.
I agree to pay all expenses related thereto and release Calvert Catholic from any liability for any
actions. Assumption of Risk and Release of Liability: I ______________ know the risks
involved in participation of the Flag Football event and in voluntary consideration of being
permitted to participate in the event. I agree to release, indemnify, and defend Calvert Catholic
and their officers, employees, staff, students, volunteers and coaches from and against any claim
which I, the participant, may have for any losses, damages or injuries that may arise from my
participation in the event.
I indicate by my signature below that I have read the terms and conditions of participation and
agree to abide by them. I have carefully read this Release form and acknowledge that I
understand it. If the participant is of minority age, the undersigned parent or guardian hereby
understands and assumes the risks for the minor. No representation, statements, or inducements,
oral or written, apart from the foregoing written statement, have been made. If any portion of this
Release Form is held invalid, the rest of the document shall continue in full force and effect.
