    The patient has a long history of schizoaffective disorder and receives monthly injections of haloperidol decanoate as prescribed. During the evaluation, she was calm and cooperative with constricted affect, but denied feelings of depression, hopelessness or helplessness. She reported some anxiety and paranoid thoughts related to her recent arrest. Her thought process was clear and she had good impulse control. She agreed to continue her monthly haloperidol injections as ordered.

    reported long standing h/o chronic mental illness( as per pt , "schizoaffective" and psych in

    the past; claimed that she receives regularly haldol dec . im qmonth , last
    inj.was given on 01-02-2014. Denied recent substance abuse hx.
    Calm,cooperative, affect constricted , denied feeling depressed, hopeless,
    helpless, somewhat anxious,guarded, some paranoid preoccupations and
    interperation , frustrated over her arrest, and stated she did not know why
    she was arrested since she felt threatened ?! denied visual , olfactory
    ,tactile hall; denied suicidal , homicidal , ideations intention and plan. Clear
    mentation. Good impulse control .
    r/o chronic schizoaffective d/o by pt's report ,
    r/o sch , paranoid type
    Pt agreed to continue haldol dec im q 4 weeks as ordered , AIMS neg , side
    effects explained, monitor ms and reevaluate
    New Medications:
    HALOPERIDOL DECANOATE 100 MG/ML SOLN (HALOPERIDOL DECANOATE) 100mg im q
    4weeks , due on 01-30-14
