Dear Fernanda:

Genco Entertainment, Inc. (the “Producer”) is currently producing a program titled “It Only Hurts
When I Laugh” (the “Program”) which will initially air on truTV. This program will feature real
life home videos and other footage of people in everyday situations and the unexpected and funny
mishaps that are caught on tape.

FERNANDA AZEVEDO AMORIM has made available for our use in connection with the
above-referenced Program certain footage in which you appear “Single Ladies Dance Fail –
Two girls doing the single ladies dance and one smacks her head onto the wall” (Clip
#43884) (the “Clip(s)”).

For good and valuable consideration, this will confirm your grant to us, our licensees, assignees,
and successors the right to use your appearance in the Clip(s), in and in connection with the
Program and the advertising, promotion and exploitation thereof, in any and all media, now
known or hereafter devised, worldwide, for fifteen (15) years. We shall have the right to produce
longer or shorter versions of the original half-hour telecast. You acknowledge that we shall have
no obligation to actually use the Clip(s) or any portion thereof or to produce or broadcast or
otherwise exploit the Program.

If the foregoing is acceptable, please so indicate by executing this consent in the space provided
below. Once executed, please return this release to me, via email: renee@bradlachmanprods.com

the Program.
