following documents: Authenticated copies of one (1) NSO Birth Certificate and one (1) Transcript of Records. (all documents shall
become PAFPMC property).

• Male or female natural born citizen of the Republic of the Philippines
• Educational Attainment: PAFROPP - Baccalaureate Degree Holder ; CS - at least 72 college units
• Must have a height of at least 152.5 cm (5’ 0”) for both male and female
• Age: PAFROPP - Must be born between 24 May 1987 to 24 May 1991 (20-24 y/o)
CS – Must be born between 14 June 1987 to 13 June 1993 (18-23 y/o)
• Single and no legal obligation to support a child or children
• Physically and mentally fit for military training
• With good moral character

1. Selected applicants (for both PAFROPP and CS) will be directed to perform and pass the Physical Fitness Test ONLY after completely
submitting the following:
- 2x2 Picture - Mayor’s clearance - C-2 or R-2 clearance
- 3x5 Picture (whole body) - Municipal Court clearance - Personal History Statement
- Transcript of Records - Regional Trial Court clearance - Affidavit of Parents Consent (21 yrs old below)
- NSO Birth Certificate - Municipal Judge clearance - Marriage Contract of Parents
- Barangay clearance - Provincial PNP clearance - Local Police clearance
- NBI clearance

2. PAFROPP applicants will be notified to report at PAFPMC for the Special Written Exam (SWE). Only those applicants who will submit
the above documents will be allowed to take the SWE.
