Learning Communities Teaching Award

NOMINATION FORM

Here’s your chance to recognize the outstanding teaching of your Learning Community (LC) instructor(s)!

This award is intended to acknowledge and celebrate an instructor whose teaching goes above and
beyond the classroom and, in so doing, encourages students (you and/or others in your LC) to expand
learning beyond the class.

In order to nominate an LC instructor,* please fill out this form and give it to your LC PAL (or drop it off at
the PAL Desk in the Murray Library) before the deadline of Friday, November 26, 2010.

* To nominate more than one LC instructor, please use a separate form for each instructor.

LC students’ nominations, LC PALs, LC Program Staff, and the LC Teaching Award Committee will review
the submissions and determine the award winner.

Deadline for nominations is Friday, November 26, 2010. Please submit your completed Nomination
