 CID form duly filled-in and signed

 2 Passport size photographs attached

 Proof for Date of Birth – School Leaving Certificate/S.S.C/ Birth Certificate

Passing certificate (where DOB is mentioned)

 For Address Proof - Any 2 documents from the following list

1. If staying on Leave and License (Notarized Agreement [old+new] + any 1
address proof of the owner)
2. Original Society Letter)
3. Electricity bill (MSEB / MSEDCL only), if the house belongs to you or your family
(surname should match)
4. Telephone Bill (MTNL only)
5. Gas Connection Slip
6. Ration Card
7. Passport (only for Mumbai)

 For Photo ID Proof - Any 1 document from the following list

1. PAN Card
2. Driving License
3. Passport
4. Election ID Card

 Do not attach Demand Draft Rs 100/- as mentioned in the form. TCS will take care of
the same.
 Candidate shall be required to visit the Local Police Station when called by Police
Authorities/TCS Representative. This is part of the Criminal verification process laid
down by the Police Authorities.
 Candidates staying in Navi Mumbai – All supporting documents need to be attested
by local Govt. Authorities / Gazetted Officers (i.e. the authorities from Navi Mumbai
only).

address proofs of the owner) OR Affidavit made on 100/- stamp paper in Pune
only
2. Original Society Letter
3. Electricity bill (Only MSEB), if the house belongs to you or your family (surname
should match)
4. Ration Card

 For Photo ID proof any 1 of the following documents would be required:

1. PAN Card
2. Driving License
3. Passport
4. Election ID Card
