I.

Background of the Client

The client’s name is Rayne Glenn Lucas, a 7 year old Grade 1 student
studying in Holy Child Catholic School. He was born on October 20, 2003
and is the eldest of two children.

II. Presenting Problem

The presenting problem of the client, according to the mother and


grandmother is more on behavioral specifically, as we quote, “masyadong
makulit”. This problem manifested since the start of early childhood.
These behavioral problems extends to:
-“hindi mapirmi sa isang lugar”
-“mapagsasabihan ngunit hindi nagtatagal ay babalik na ulit sa dati”.
-While watching TV, the client seems to have no focus and has lots of
unnecessary movements.
-Hyper active

III. Assessments of the students development

Physical
The client has an endomorphic body type. It is manifested by his broad
shoulder and his healthy body. The mother also added that his son was
not prone to sickness. Furthermore, during observation, the assessor
noticed that the client is active in playing outdoor games as well as indoor
games and was not physically lame.
Moreover, before entering pre-school comparing to other children of
his age he already mastered bladder control.

Mental
The client was a consistent honor since he started schooling. He also often
receives awards such as “Best in Mathematics, Best in Filipino, Best in
Science, Best in Language and an Over-all Fourth Honor.” In addition, he
also receives a quarterly certificate of recognition as Second Honor.
Furthermore, the client proudly said that his favourite subject is “Math”
and with that he even gave us a sample of his last lesson in the said
subject which is “skip counting”.

Social
At first, Rayne is a shy, quiet boy. He was still “nangingilala” then when he
gets comfortable with us, Rayne answers our questions attentively and do
what we asked him to do. When he was asked questions, it takes some
time for him to answer us back.
