IMPORTANT INSTRUCTIONS (To be retained by Pilgrim)

(PLEASE READ THE INSTRUCTIONS CAREFULLY, IT IS FOR YOUR OWN BENEFIT AND CONVENIENCE)
1. INTERNATIONAL PASSPORT: The Kingdom of Saudi Arabia has intimated that Haj Visa will be endorsed on International Passport only. Hence
every Pilgrim must have International Passport. The Pilgrims are requested to submit the Haj Application Form alongwith photo copy of International
Passport / Regional Passport Office receipt to the concerned State Haj Committee on or before 30th April, 2011.
2. HOW TO APPLY:
I. The Application Form (in duplicate) should be filled in ENGLISH BLOCK LETTERS with blue / black pen only. Please don't use carbon.
II. Latest 3.5cm X 3.5cm passport size front pose with white background colour photograph to be pasted on each application form.
III. Reserved category:
a)- Rejected last three years: The competent authority has decided to directly confirm the applications of the Pilgrims whose applications
have been rejected continuously for last three (3) years, during Haj 2008, 2009 and 2010. The applicant along with his co-pilgrims if any has t o
indicate the cover number in the relevant column. The application, which does not match with previous year Cover Number will be
registered as a fresh application and will be included in General Category. Option once made at the time of registration, will not be changed
at any cost.
Those applicants who have been selected during previous 3 (three) years, but cancelled the journey on their own, will not be registered
under Reserved Category and shall be considered as fresh and registered in General Category.
b)- PNR / No Visa: Applicants who were selected last year i.e. Haj 2010 and deposited full amount but could not proceed for Haj 2010 due
to non issuance of passport /visa will be given benefit of reserved category if applied with details of last year cover number subject to
confirmation by Haj Committee of India.
(Note: - Applicant who applied under reserved category must hold valid International Passport and submit the photo copy of
passport. No new applicants should be added to the cover.)
IV. Head of cover: The head of the cover should be male applicant only.
V. Accommodation: The applicants are required to indicate their option for accommodation and tick at relevant column as per categories:-
I - GREEN, II - WHITE and III - AZIZIA
Note: - If failed to tick the accommodation category it would be presumed as AZIZIA.
The choice made and expressed in the Application form with reference to Accommodation shall be final. No request for its change shall be
entertained subsequently.
VI. Applicant Bio Data: Furnish correct name and other information as per International passport.
VII. Nominee: The name and other particulars of the nominee, who could be contacted in case of emergency in India/Saudi Arabia, should
be “ legally maintainable” to be recorded distinctly at the appropriate place in the Application Form.
VIII. Mehram: For female pilgrims only (The Lady Pilgrim should be accompanied with Sharai Mehram and Mehram should not be a
Repeater.)
IX. Account Details: The details of bank account of the applicant may be furnished in the form. Photo copy of cancelled blank cheque may be
attached with form.
X. Payment: Each Application Form is required to be accompanied by remittance of Rs.200/- (Rupees Two Hundred Only) per applicant by
pay in slip into the Current Bank Account No.31634038682 maintained with State Bank of India through core banking system towards n o n
refundable processing fee.
XI. Meeqat (JOHFA): Shia Pilgrims who opt JOHFA as Meeqat should mark the same in the Haj Application Form.
3. Every Pilgrim is required to sign the solemn declaration and Undertaking incorporated on the reverse of Haj Application.
4. Application Form duly filled in should be attested by any one of the following with reference to applicant's area/ locality.
MP; Member Haj Committee of India. MLA, Member State Haj Committee, Corporator, Principal of recognised School, Member District Board, Member
Zilla Parishad of District of the applicant, General Manager of Banks of Government Undertaking, Tehsildars, Gazatted Officer, Inspector Incharge
of Police Station, President or Sarpanch of Village Panchayat, BDO, VDO, SDM of the place of residence of the applicant.
5. The certificates of physical fitness, particulars of blood group and case history are required to be recorded by the competent authority in the relevant
columns.
6. The following persons are not entitled to make application for Haj 1432 (H) 2011,
i. Those who have performed Haj during last 5 years viz. during the block year 2006-II to 2010. This restriction applies even for Haj-e-Badal.
ii. Those suffering from tuberculosis, Aids or any other communicable diseases or crippled, handicapped, lunatics or otherwise physically
incapacitated.
iii. The Lady Pilgrim should be accompanied with Sharai Mehram and that Mehram should not be a Repeater.
7. Ladies in advanced stage of pregnancy are advised in their own interest not to undertake Haj journey.
8. The Application Form should be filled strictly in accordance with the Guidelines formulated for Haj 1432 (H) 2011. A copy of which can be obtained
from the concerned State Haj Committee, free of cost.
9. Please ensure that all particulars recorded in the Application Form are true and correct to the best of your knowledge and that you have affixed y o u r
signature/thumb impression at the appropriate places in the Form. It needs to be noted that the false declarations recorded, untrue statements made or
suppression of facts shall lead to forfeiture of money deposited, apart from the rejection of Application. Incomplete, incorrect applications will be summarily
rejected.
10. LAST DATE: The Haj Application Form with copy of valid International Passport/ Regional Passport Office receipt, copy of pay-in-slip, copy of cancelled
blank cheque and copy of residence proof should be submitted to the concerned State Haj Committee on or before 30th April, 2011.
11. The Application Form should be submitted through concerned STATE HAJ COMMITTEE only.
12. The Pilgrims are advised to retain photo copy of the Application Form instructions and Haj Guidelines.
13. COVER SIZE: A maximum number of Five persons comprising the family members or close relatives can make Application in a single cover. Minimum
size of cover is one. The head of the cover should be male applicant. It is in the interest of the Pilgrims to ensure that only family members having blood
relation are included in the cover. The head of the cover should be responsible for all the payments of the persons included in the cover. An alien person
should not be included in the cover.
14. Please note that Haj Committee of India has no agents. Please do not pay any extra amount to any individual / organization or travel agent.
15. EMBARKATION POINT: The Pilgrims are compulsorily required to travel from their natural Zone-wise Embarkation / Disembarkation Points as per the
table given on cover page of the Form and should be in conformity with the address of the Pilgrim.
16. AFTER QURRAH: Provisional selection in Qurrah (Lottery) system shall be communicated to the intending Pilgrims by the respective State H a j
Committees. Thereafter selected Pilgrims will have to submit valid International passport with photograph stapled on cover page and deposit copy of pay-
in-slip for Rs.31,000/- towards advance Foreign Exchange and Miscellaneous dues to State Haj Committee on or before 15th June, 2011.
17. FOREIGN EXCHANGE : Balance Foreign Exchange and Air Fare amount to be paid on or before 1st August, 2011. Category-wise rate will be
announced later.
18. CANCELLATION AND REFUND: Once cancellation is marked on the request of pilgrim, it will not be revoked on any ground. The applicants are informed
that the cancellation of journey and refunds out of the advance Foreign Exchange deposited will be governed as per the norms laid down in the
Guidelines for Haj 1432 (H) 2011.
19. HEALTH REGULATION (Medical Card): Medical health card should be obtained from concerned State Haj Committee. Take influenza
inoculation (ACYW 135)10 days before Haj journey. The said certificate should kept with you during journey.
20. LEGAL STATUS OF THE PILGRIM :
a. All the Pilgrims are informed that the Haj Committee of India, constituted under the Act of Parliament renders service to the Pilgrim
without any consideration. The payment of Rs.200/. made by pilgrim is towards processing charges of application. The Haj Committee of
India renders its services to the Pilgrim free of charge. The Haj Committee of India, as such does not come within purview of Consumer
Protection Act 1986. The Pilgrim, therefore, can not claim any compensation against Haj Committee of India under Consumer Protection
Act.
b. JURISDICTION : The Court of Greater Mumbai only shall exercise Jurisdiction in all matters of dispute, if any.
