extend to any and all supplemental materials that may be submitted by our organization
to the Commission relating to this project.

I fully understand it is my duty to report to Commission staff any material changes in the
organization and/or the project, including but not limited to: non-profit status, property
interest, project scope, costs, and timelines, funding sources and amounts, and
construction administration team.

I acknowledge receipt of draft administrative action no. A-07-04 and understand that the
disbursement of the state appropriation is contingent upon approval of administrative
action no. A-07-04 by the Commission and satisfactory completion of the conditions set
forth therein.
