SUBMIT ANY FURTHER REQUIRED DOCUMENTATION AND
ACKNOWLEDGEMENT OF RESOLUTION NO. R-07-44

I hereby certify that all information contained in the Project Registration and Detail Sheet
submitted to the Ohio Cultural Facilities Commission (the “Commission”) is true and
accurate. I further certify that all attachments and exhibits submitted to the Commission
are true and accurate to the best of my knowledge and belief. This certification will
extend to any and all supplemental materials that may be submitted by our organization
to the Commission relating to this project.

I fully understand it is my duty to report to Commission staff any material changes in the
organization and/or the project, including but not limited to: non-profit status, property
interest, project scope, costs, and timelines, funding sources and amounts, and
construction administration team.

I acknowledge receipt of draft resolution no.R-07-44 and understand that the

disbursement of the state appropriation is contingent upon approval of resolution
no. R-07-44 by the Commission and satisfactory completion of the conditions set forth
therein. I further agree that, if requested by the Commission, a representative from
National Underground Railroad Freedom Center will be present at the November
29,2007 Commission meeting to answer any questions related to the project or
organization.
