Scholarship
• The Curtis Middle School PTA will award two scholarships to be presented
at the Senior Awards Ceremony for up to $500 each.

Eligibility for Application

• All college bound Allen High School graduating seniors who attended
Curtis Middle School (CMS) at least 1 full year (either 7th and/or 8th grade)
are eligible to apply.

Requirements for Application

• Type or print application form and return with essay described below.
• Submit two names of reference:
 One from an academic source such as high school counselor,
teacher of a major subject, or house principal
 One from a non-academic source outside of your family
• Submit a 350 word typed essay with the following focus: “Please share
with us an experience you had with a teacher or an organization at
Curtis M.S. that inspired you to be a leader in High School.” (Band-
Section Leader)
• Staple and return completed application and essay in a sealed envelope to
the office of

April 15, 2011. No late or incomplete applications will be
