 Grading System
• Midterm Grade = 60% performance rating
+ 40% compliance to requirements
including attendance to meetings
• Final Grade = 60% performance
evaluation + 20% Midterm Grade + 10%
Final Report + 10% compliance
 Midterm Requirements:
• Confirmation Letter ( original to OIR, photocopy to adviser )
• Training Agreement ( original to OIR, photocopy to adviser )
• Waiver ( original to OIR, photocopy to adviser )
• Logbook ( with updated entries: trainee information sheet, company
information sheet, daily activities written by trainee, remarks by trainor,
trainor’s signature )

• Performance Evaluation ( pp 94-95 of logbook with remarks

by trainor )

• Admission Slip
 Final Requirements:
• Updated Logbook
• Performance Evaluation ( get form from office, should be
filled out by trainor and should be placed in a long signature-sealed letter envelope)

• Final Report (format found in the logbook)

• Certificate of Completion (with number of hours rendered)
• Admission Slip

 Deadlines:
• 1 week after midterm exam
• 3 weeks after final exam

 INC – Incomplete Grade
• Given to reasonable cases only
• 1 semester to complete otherwise failed
so, must re-enroll
• Can not enroll OJT 2 unless INC is lifted
through completion of requirements
therefore, it should be processed before
end of enrolment
