Check-Off
1.___ All employee (certified and classified) check-out sheets completed and turned into the District
Service Center.
2.___ All employee (certified and classified) evaluations completed and turned into the District Service
Center.
4.___ Personnel records complete and submitted as required- certification of absence, time sheets; for
12-month employees leave requests for summer vacation and/or personal leave, etc.
Reminders
____2. Mail- summer mail needs to be picked up at the District Service Center every two weeks.
____3. Financial records for audit- completed/bound & submitted to District Service Center by July
15th.
____4. Petty cash- total amount returned to District Service Center by check prior to June 11th.
____6. Your summer schedule– if you go out of town for extended time let us know who
will cover for you in case of an emergency.
___ 8. Make sure all Title I, LEP, results and reports are given to Sandra Miller before leaving for the
summer. Safe & Drug Free results and reports are given to Sanie Baker. Special Education
Discipline reports must also be completed. Most of these are now online.
Please complete and return to the District Service Center by June 5 th of each school year.
During the summer months, your check will not be mailed, but will be deposited directly with the stub
available online at www.minidokaschools.org under iVisions employee portal. If you “opted out” of direct
deposit, you can pick up your check on the official pay date at the District Service Center (District Policy
# 810.96).
Reminder: All credits earned for educational experience must be turned into the District Service Center on
or before September 15th of each year.
