com CONSIGNMENT AGREEMENT This agreement is made on ______ / _______ / 20 _______ between _______________________________ , herein referred to as "Seller", and Chuckz N Rosez by April Renee. Whereas Chuckz N Rosez by April Renee wishes to sell (description of item(s)) by consigning said item(s) (*See attached inventory sheet/ price list) to ________________________________ for sale, it is understood: TERMS & CONDITIONS That Seller agrees to display item(s) in a prominent place in his/her establishment. Items will be merchandised in a manner that best showcases the designers work, both inside of store and in the store window display. That Seller will make every attempt to obtain the best possible price for the consigned merchandise and will accept no less than the dollar amount as the purchase price, designated by the designer, unless authorized to do so by Chuckz N Rosez by April Renee The seller agrees also to not remove product tags and logos from the merchandise. PAYMENT The Seller is entitled to retain 30 % of the purchase price of each sale item, for his/her efforts to promote and accomplish a successful sale. I.E. Merchandise price: $60 (Designer 70%: $42 / Seller 30%: $18) Chuckz N Rosez by April Renee will be paid on the last day of each month for any items that has been sold. The seller will also 1

The Seller represents that he maintains insurance for theft and damage, and that the consigned merchandise will be covered by said insurance while it is in his possession. Chuckz N Rosez by April Renee agrees to leave the merchandise with Seller for a minimum of ____________ months, to effectively promote and sale to the sellers clientele. Should the merchandise remain unsold at the end of the consignment period and an election be made by the Chuckz N Rosez by April Renee to remove said merchandise, any costs incurred by the delivery of same to Chuckz N Rosez by April Renee shall be borne by Chuckz N Rosez by April Renee.
