Dear Madam: A Pleasant Day! We would like to take such opportunity to further practice and develop our skills in Intravenous Therapy learned from our 3 day basic IV Therapy Training Program held in your hospital on April 27-29, 2012. This letter of intent indicates our interest and willingness to be trained under your approval, guidance and counsel as IV therapy nurses in your institution for the completion of 3+3+1 requirements of ANSAP. The completion of the cases is scheduled within May 30 to June 6, 2012 under Mrs. Imelda P. Akut as the preceptor. It would be a privilege to render our service in Northern Mindanao Medical Center because it is a tertiary training medical center, reflective of a highly specialized level IV institution.
