FAMILY CARE PLAN

Introduction

A family is a group of people affiliated by consanguinity, affinity, or coresidence. In most societies it is the principal institution for the socialization of children. In the family, a researcher can identify problems such as health problems, financial problems, social problems, etc. Such problems are often unidentified due to knowledge deficit of the family to the signs and symptoms of the disease and the treatments. Knowing that not all people in a certain family are financially capable and educated, nurses should take extra measures especially in conducting teachings in the family.

The Public Health Nurse educates the family and help them identify potential hazards to the family to help in the improvement of the familys lifestyle. A nurse compiles and analyzes all data gathered and develops a Family care plan to systematically and efficiently deliver a family care plan with considerations in the financial capability of the family, willingness, competency, etc.

This case study is focused on one of the Manto family in 0164, Relocation 5, Pulong Sta. Cruz, Sta. Rosa Laguna. The researcher has encountered health threats as well as health deficit in the family.

I. Family structure, Characteristics and Dynamics Family Manto is a nuclear type of family. They are residing 0164, Relocation 5, Pagoyo, Sta. Rosa, Laguna. The family is headed by Mr. Sopronio Manto (51 yrs.old), elementary graduate. Mrs. Arcelie Manto(43 yrs. Old) was able to finish until level 5 of elementary school, Jason Manto ( 29 yrs.old ) high school graduate, married but staying with his parents and away from his own family, Joey Manto ( 26 yrs. Old) High school graduate, married and away from his own family but staying with his parents, Jonathan Manto (21 yrs.old) single and works as a landscape worker, and Jaymarose Manto (9 yrs. Old) Grade 3 elementary student. The family communicates and interacts with each other well. Decision making is done by Mr. Sopronio Manto, head of the family.

Socio-economic and Culture characteristics Mrs. Arcelie stated that his husband earns P350.00 per day as a

construction worker and gets approximately P6,500 per month. HerThe family has their own house. Mrs. Arcelie is a housewife and she does all the house chores. The family is attending church services regularly but they are not participating in community activities.

III. Home and Environment The family has their own house but their lot is rent-free. The house is attached to another house and the structure is makeshift with mixed flooring and it is compose of 1 bedroom, 1 window. The family shares a bedroom

together. Their source of light is from a fluorescent light bulb powered by electricity. They get their drinking water from a store that sells purified drinking water which is in a jug. But their water for cooking, cleaning the house, laundry, is from a pump outside their house. They also store their water for cooking in jugs with cover and their water for household in drums with cover as well. They cook their own food and store it on the table in a cupboard. Their garbage is stored in a sack with cover and it is collected every week by a truck. They have a septic tank for their human wastes which is shared by 2 families. They have a cellular phone as their method of communication and they utilize public transport for their transportation.

IV. Health status of each member Jonathan is a smoker and drinks alcohol occasionally with his brother Jason but Jason does not smoke. The family does not often go for medical checkup. Everyone in the family is physically fit. When health problems occur, they seek advice from a friend.

Values and practices of Health promotion and Maintenance and Disease

Prevention The family is not too much conscious of their health. The family had verbalized, that they have an inadequate rest and sleep. They do not have any exercise. The familys main relaxation activity is watching TV.
