Providing health and hope to the hurting Commitment to Care Program

With our room sponsorship opportunities, you and/or your company can be a part of the healing process. Health and Hope Clinic provides primary and specialized medical care to the uninsured and under-served members of our community. Jesus said, You will always have the poor among you (Matthew 26:11). Jesus also said, Let your light shine (Matthew 5:15) It is our calling to serve our brothers and sisters who need us most. Our program promotes a long-term healthy lifestyle with classes to educate our patients on how to manage their illnesses Eligible patients are at or below 200% of the Federal Poverty Line, have no insurance coverage and do not qualify for federal funding, i.e. Medicaid/Medicare 80% of our patients are hard-working, employed members of our community and simply cannot afford insurance coverage In 2011 for every $1 donated, we provided $12 in medical services With 190 volunteers, 80 of whom are medical providers, we will continue to serve our neighbors with the following services: General Primary Care Specialty Care o Rheumatology o Gynecology o Dermatology o Chiropractic o Neurology Acute care for non-emergent conditions Office surgical procedures In-office laboratory services Prescription medications and pharmacy services Preventative care In 2011, we served 2,000 patient visits, providing more than $1.7 million dollars in medical services. Recently, the Volunteer Services Coordinator for the Florida Department of Health had this to say about our clinic: I continue to believe that you guys are the Gold Standard for volunteer clinics, and I wish I could bring some of my other clinics in to show them how it should be done. I am thankful for you.

Care Packages

Exam Room Package-$2,500

Only four rooms available; one entity per room Visible partnership with Health and Hope Clinic A plaque will be placed on the door of the room, prominently displaying your name, certifying your adoption of the room. You will receive our quarterly newsletter and be recognized as a Commitment to Care Partner by name and logo. Company logo and information will be displayed on the clinic website, including clickable link to company page. Company logo and information will be printed on collateral material related to the clinic, i.e. clinic brochures You and your employees/friends will be invited, but not required, to assist with any upgrades to your room: i.e. painting, furnishing, etc. You will have the first right of renewal on your room at the end of your commitment, should you wish to continue your sponsorship.

Laboratory Package-$1,500
One entity only

Visible partnership with Health and Hope Clinic. Exclusive sponsorship to lab room A plaque will be placed on the wall in the laboratory, showing your support as a Commitment to Care Partner by name and logo. You will receive our quarterly newsletter and be recognized as a Commitment to Care Partner. Company logo and information will be displayed on the clinic website. You will have the first right of renewal on your room at the end of your commitment, should you wish to continue your sponsorship.

Waiting Room Package-$1,000
Limited to five entities, ONLY FOUR AVAILABLE as of 8/14/12

Visible partnership with Health and Hope Clinic. Shared sponsorship between 5 individual entities. A plaque will be placed on the wall in the waiting room, showing your support as a Commitment to Care Partner by name and logo. You will receive our quarterly newsletter and be recognized as a Commitment to Care Partner. Company logo and information will be displayed on the clinic website. You will have the first right of renewal on your room at the end of your commitment, should you wish to continue your sponsorship.

Caring Heart Package-$250

Unlimited

Visible partnership with Health and Hope Clinic. Donor name displayed on Caring Hearts page of website Donor name will be displayed on Caring Hearts plaque in clinic Donors receive our quarterly newsletter and be recognized as a member of our Caring Hearts Society
